- Capability Questionnaire This is a Sources Sought Notice to determine business size and market capability for an upcoming competitive requirement for Pre-Employment medical Testing Support Services for Customs and Border Protection Officers and candidates. This is not a request for Proposals and no solicitation is being currently issued. Please respond by e-mail only to patricia.startzman@dhs.gov. No phone calls will be returned at this time. The NAIC code is 62111, Medical Evaluation Testing in Physician's office (or Clinic). Small Business size is $10 Million or less annual revenue. An eligible small business must qualify under NAIC code 612111 as small to be considere small for this upcoming competition. The pupose of the future requirement will be to obtain services for scheduling of medical tests for an estimated 6-10,000 or more individuals annually via electronic receipt and transfer of scheduling and medical testing data/requiremetns. The medical tests must be administered by medically certified individuals in medical facilities that meet the federal/state criteria for medical procedures. The testing services will be required within sixty miles of a candidate's residence or withing 50 miles of an employees worksites (primarily ports of entry) This can be considered nationwide to worldwide to include all 50 states, Guam, Puerto Rico, and the Southern Border of Canada. Medical Tests performed will inculde urine tests (to determine, if drug free); Tests required for termination of Physical fitness for return to work for Law Enforcement Duites or similiar type of position. The vendor must be capable of providing an automated order process iwth a da tabase for storing test information securely as per federal regulations. Potential contract must also have the caability for programmed data to be transmitted to CBP. Prefereably have past performance on providing medical evaluation tests nationwide and at times worldwide. This sources sought is for information purposes only to determine if this requirement can be set aside for small business. Questions will be answer only through e-mail at this time; no vendor will be invited for sales presenation or capability briefing. Any vendor not completing the attached form will be considered not to be capabile of meeting the required criteria to be include in small business set aside decision process.
