Only electronic submissions will be accepted. EVALUATION FACTORS Offerors will be evaluated to the extent they can provide the items (or equal) listed in the price schedule in Section B. The Government will award to the lowest priced, responsible, technically acceptable offeror. An "or equal" item must: 1.Must be compatible--form, fit, and function--when using with the bioMerieux BacTAlert. 2.Meet the standards in World Health Organization, Guidance on regulations for the Transport of Infectious Substances 2011-2012, document WHO/HSE/IHR/2010.8 3.Meet standard phlebotomy procedures for all hospitals, 1611 Latex Allergy Guidelines and Latex Management in Hospital Settings 4.Must be fit for phlebotomists and nurses to collect blood cultures. 5.2 Dynarex Alcohol Prep pads (1103) must be standard package size for individual use (approximately 2inch x 2.5inch per single pad (need 2) or 4inch x 2.5inch per 2ply package) 6.1 Medline Tourniquest 1" rolled banded LF (DYND75020) must be 1inch width x 18inch length 7.2 BD Syringes, 10ml luer lock (309604) must be Luer-Lok Tim, 10 ml draw, sterile. Each syringe will be labeled with "DRAW 20 Ml". 8.1 2% Chlorhexidine (PDI) Chloraprep Swabstick (S32450) must fit in final packaged kit and be no bigger than 6" x 1". Prep stick must be saturated with 2% Chlorhexidine. Each PDI swabstick will be labeled with "SCRUB 30 SECONDS. LET DRY 2 MINUTES" 9. 1 Tyco Angel wing female adaptor BacTAlert blood culture device (225241) must be compatible (form, fit, and function) with BacTAlert collection bottle. 10. 1 Dynarex 2x2 gauze (3362) must be Standard gauze package of 2 inches by 2 inches 11.1 Dynarex Bandaid (AER1018) (non latex) must be Standard size (approximately 3inches x.5 inch) 12. 1 PIP Instruction (custom provided by VA Palo Alto HCS, see attachment 1) must be color printed on 4 inch x 6 inch card placed on top of all components as the first item seen. Offeror must provide a copy of vendor instructions with offer (see attachment 1). 13. 1 Southpack Tray (TR-BMX2) must have a footprint that can contain components of the kit (4' x 6" x 2"). FAR 52.211-6 Brand Name or Equal (Aug 1999) (a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. (b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must- (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by- (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. (c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation.
