- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation nor a request for quotes. The purpose of this announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified sources, including their size classification. This Sources Sought Announcement shall not be construed as a commitment by the Government to issue a solicitation or award a contract, and this announcement does not restrict the Government to a particular acquisition approach. The Government shall in no way be bound to this information if any solicitation is issued. The NAICS code for this acquisition is 562112. The Department of Veteran Affairs (VA), Michael E. DeBakey Medical Center is seeking information on a potential contractor to provide all services to assess, update, and provide enhancements to sustain the Michael E. DeBakey VAMC (MEDVAMC) facility's pharmaceutical waste management program, in accordance with MCP 138S-012, EPA, OSHA, DEA, CDC, NRC, ISO 14001 and other Pharmaceutical Industry Best Management Practices. Contractor will also be required to provide Pharmaceutical waste management program services to MEDVAMC pharmaceutical dispensing operations/facilities, including one Domiciliary and approximately nine CBOCs Locations are located in the following cities of Texas: Galveston Island, Texas City, Lufkin, Beaumont, Conroe, Richmond Rosenberg, Lake Jackson, Katy, and Tomball. All Pharmaceutical waste services shall be in compliance with MEDVAMC contract specifications, and Federal, State, and Local Pharmaceutical waste regulations. Sources interested in this notice shall submit the following information: 1. Company name, address, point of contact, phone, email address, DUNS number cage code, size of business pursuant to North American industrial classification system (NAICS) 562112. 2. Evidences of applicable business permits, licenses required by the Federal, State, and Local Pharmaceutical waste regulations to provide the aforementioned services in all geographic areas described above. Responses to this notice shall be in writing by e-mail addressed to kieu.dinh@va.gov. with "Pharmaceutical Waste" in the subject line. All responses must be received in writing no later than 4:00 PM CST. December 22, 2011. Telephone responses will not be accepted.
