Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Synopsis: This is a combined synopsis/solicitation for non- personal services the establishment of a Physical Therapy Pool Service Maintenance Blanket Purchase Agreement (BPA) prepared in accordance with the format in FAR Subparts 13.1 Simplified Acquisition procedures as supplemented with additional information included in this notice. This BPA is being established to support the Fort Belvoir Community Hospital, Fort Belvoir, VA 22060. This announcement constitutes the only solicitation, quotes are being requested, and a written solicitation will not be issued. This solicitation document incorporate FAR provisions and clauses through Federal Acquisition Circular (FAC) 2005-54. The solicitation reference number is W91YTZ-12-T-0007. This acquisition will be a 100% small business set-aside for the North American Industry Classification System (NAICS) code: 561790. The business size standard is $7,000,000.00. This request for quotation is for Physical Therapy Pool Maintenance Services Fort Belvoir Community Hospital, Fort Belvoir, VA. (Please Note: This solicitation is set-a-side for Small Businesses (SB) and is limited to SB located within 50 miles radius to Fort Belvoir, VA 22060-5901.) 1.)DESCRIPTION OF AGREEMENT: This is a pre-priced Blanket Purchase Agreement (BPA) for non-personal services. under this BPA, the Contractor shall provide all materials to perform the maintenance on the pool with dimensions 24.4 by 41.4 total sq ft 1010.6. The following is a summary of work: The Contractor shall service the pool/chemical room to maintain good operating conditions no more than two (2) times a week. The Contractor is responsible for cleaning up after themselves and their equipment, leaving the facility as they found it. Plus one (1) annual cleaning of the pool, drain, rails, gutters, and power wash the deck. The contractor shall remedy any deficiencies noted by the Government under the, BPA to the extent identified in the BPA's scope of work. CONTRACTOR will render the swimming pool operational by completing the following services: a.Inspect the pool, its premises, and plumbing in accordance with: *TB MED 575 *ASME A 112.2.19.8 b. Check all equipment. c. Reclamation of pool water will be attempted as primary opening procedure, drain and acid cleaning the pool (if necessary). d. Check inventory. e. Contractor will advise Pool Staff of needed repairs to the pool upon execution of this BPA. CONTRACTOR shall perform the following services: 1. Check complete pool chemistry. a.Bacteriological: HCP not to exceed 200 CFU per 1.0 ml b.pH:7.2-7.8 c.Free Available Chlorine: 2-3.5 ppm (mg/L) d.Max Chlorine > 4PPM 2. Balance pool chemistry as needed. 3. Check on all skimmer, filtration equipment. a. Del Genesis Ozone Sanitation System b. Triton II Commercial Sand Filter c. Polaris Watermatic 4. Backwash/clean filters as needed. 5. Record all chemistry levels and filter adjustments. a. On DA Form 7267-R 6. Moreover, services will include (if not redundant with items above): - Perform pump and filter manufacturer's recommended maintenance - Vacuum/clean pool - Operation checks - Backwashing - Water chemical treatment - Chlorinating and super chlorinating - Log all visits All materials and equipment that is not the property of the Government shall be removed from the work site at the completion of all service performed and the area shall be left in a neat and orderly condition. This service will be ordered by the Contracting Officer (or authorized representative of the Contracting Officer) during the five (5) year duration of this Blanket Purchase Agreement during the period of January 2011 thru January 2016. All work and services performed under the BPA shall be conducted on-site. 2.) EXTENT OF OBLIGATION: The Government will be obligated only to the extent of authorized purchases actually made under this BPA by authorized personnel. 3.) PRICING: Prices quoted for the Pool Service Maintenance shall be as low as or lower than those charged under the supplier's most favored customer for comparable quantities under similar terms and conditions in addition to any discounts for prompt payment. 4.) PURCHASE LIMITATION: Individual Call Orders placed under this BPA shall not exceed a total combination of $25,000.00 per call. 5.) NOTICE OF INDIVIDUALS AUTHORIZED TO PURCHASE UNDER THIS BPA AND DOLLAR LIMITATIONS: * A list of names of individuals authorized to purchase under this agreement, identified by name and title of position and by name of: North Atlantic Regional Contracting Office (NRCO) Fort Belvoir, VA 22060-5901 * The period of any discounts will commence on the final date of the billing period or on the date of receipt of invoice for all deliveries accepted during the billing period, whichever is later. 6.) PLACE OF PERFORMANCE: Fort Belvoir Community Hospital (FBCH) 9300 DeWitt Loop, Fort Belvoir, VA 22060-5901 7.) DISCOUNT TERMS: As stated on individual invoices. 8.) PAYMENTS: Payments will be made by: GOVERNMENT PURCHASE CREDIT CARD (Alternatively, orders may be paid by Wide Area Work-Flow (WAWF) depending on dollar size of order)). 9.) COMPLIANCE WITH APPLICABLE FEDERAL, STATE AND LOCAL REQUIREMENTS: In performing work under this agreement, the contractor shall comply with all relevant federal, state and local statues, ordinance, laws and regulations. 10.) PROVISIONS ON CONTRACTING OFFICERS AUTHORITY: The Contracting Officer is the only person authorized to approve changes in any of the requirements under this agreement and notwithstanding any provision contained elsewhere in this agreement, said authority remains solely with the Contracting Officer. In the event the contractor effects any such change at the direction of any person other than the Contacting Officer, the change will be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred as a result thereof. 11.) CANCELLATION: Subject BPA is cancellable by either party within a thirty (30) day written notice. Clauses may be accessed electronically in full text through http://farsite.hill.af.mil.
