- Statement of Work This is a combined synopsis and solicitation for the following items: CLIN 0001 - The contractor shall provide a base of 22 portable toilets with hand sanitizer and weekly cleaning/maintenance of the toilets to the 388TH RANS DET 1 an additional 8 toilets with hand sanitizer will be provided when needed to meet mission requirements (minimum 48 hour notice will be given when additional toilets are requested) for a total of 30. Toilets will be located and maintained throughout the Utah Test and Training Range (UTTR). Movement of toilets will be to locations on the UTTR to meet 388th RANS DET 1 mission requirements. CLIN 0002 - additional 8 units priced each Quantity: 22 units for 1yr, 8 additional units when/as necessary This contract is being considered for 1yr plus 4 options, please provide pricing accordingly. Evaluation will be based on the "base price" of the 22 units. There are no guarantees for the additional 8 units, please price seperately in your bid. See attached specifications sheet Due to the complexity of this requirement, Offeror must attend scheduled "Site Visit" before submitting offer. Site Visit will be performed at Dugway range near Toole, UT on Jan 4th @ 10:00am. To allow access to Dugway, the names of visiting personnel must be submitted one day prior to the site visit (3 Jan). If your information is not submitted one day prior please do not attend, you will not be given access to Dugway. Delivery Date: Jan 11-14, 2012 This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and therefore quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) and Online Representations and Certifications Applications (ORCA) http://www.bpn.gov/. This solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-53. The following provisions and clauses apply to this acquisition: SOLICITATION PROVISIONS 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (IAW FAR 12.301(b)(1)) JAN 2005 Addendum to 52.212-1: Paragraph (b), entitled "Submission of Offers,': Due to the complexity of this requirement, Offeror must attend scheduled "Site Visit" before submitting offers. Site Visit will be performed at Dugway range near Toole, UT on Jan 4th @ 10:00am. To allow access to Dugway, the names of visiting personnel must be submitted one day prior to the site visit(3 Jan). If your information is not submitted one day prior please do not attend, you will not be given access to Dugway. Please prepare bids with two separate line items. One line item for the 22 base units, the other line item as a single price per additional units. Offers may be submitted via electronic mail. Submit signed and dated offer at or before 0800 MST, 09 JAN 2012. Send offer via electronic mail to david.decker@hill.af.mil. Submit quotations on company letterhead or pricing document. Quotes must include as a minimum: a) company contact information, b) price breakout for the requested line items, c) CAGE Code (from CCR registration), and d) any other information relevant to the proposal. The government reserves the right to make award on the initial quote without discussions. No partial bids, only complete offers will be considered. 52.212-1 Paragraph (c), entitled ‘Period for Acceptance of Offers': The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers. 52.214-7 entitled "Late submissions, modifications, revisions, and withdrawals of offers,': EVALUATION-COMMERCIAL ITEMS(IAW FAR 12.301(c)(1)) 52.212-2: Evaluation -- Commercial Items, The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforms to the solicitation requirements. Quotes will be evaluated on lowest price technically acceptable. Evaluation will be based on the base price of the 22 units per month.
