THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE; THE VA IS NOT SEEKING PRICING OR OFFERS. The Department of Veterans Affairs, Network Contracting Activity-8, in support of James A. Haley Veterans Hospital, is performing market research for: Software Support Subscription. This requirement is for Software Subscription to provide for Phone Technical Support via Phone, Fax or Email, Application Software Upgrades on CD and the ability for Nuclear Medicine Staff to make or burn CD's from the viewing application for easy data retrieval and referral. The Systems to be covered are Delta viewer EE# 67680, 72627, and 74936 all located at James A. Haley (VA) Hospital, Nuclear Medicine. This market research is to compile data regarding: (1) The availability and capability of qualified small business concerns; (2) Identify if there are sufficient number of qualified Service-Disabled Veteran-Owned, Veteran Owned, Small Business, HUB Zone 8(a), Women-Owned, or Small Disadvantaged Business concerns relative to the subject North American Industry Classification System (NAICS) code. Responses to the information requested will assist the Government in determining the appropriate procurement method, including any set-aside decision. Respondents should clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them. Furthermore, interested sources must also meet all requirements of Federal, State, and Local Municipal codes regarding operations of this type service. The NAICS code assigned is: 541512, Computer Designs Services and the small business size standard are 500 employees. Not responding to this Sources Sought does not preclude participation in any future or potential solicitation, or Request for Proposal. It is the intent of James A. Haley Veterans Hospital to use the information gathered for market research purposes only. INCLUDES ONE (1) YEAR SOFTWARE TELEPHONE SUPPORT AND ALL SOFTWARE MAINTENANCE. STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Software Support Subscription. 2. Scope of Work: Software Subscription to provide for Phone Technical Support via Phone, Fax or Email, Application Software Upgrades on CD and the ability for Nuclear Medicine Staff to make or burn CD's from the viewing application for easy data retrieval and referral. The Systems to be covered are Delta viewer EE# 67680, 72627, and 74936 all located in Room A333-01 Nuclear Medicine, James A. Haley (VA) Hospital. 4. Type of Contract: Firm and Fixed Price C. GENERAL REQUIREMENTS 1.The VA shall not provide service manuals or service diagnostic software to the Contractor. The Contractor shall obtain, have on file, and make available to its FSE(s), all operational and technical documentation (e.g., operational and service manuals, schematics, and parts lists) which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name, and/or the manuals themselves shall be provided to the CO upon request. Any charges for parts, manuals, tools, or software required to successfully completing any services required, and it's agreed upon price unless specifically stated in writing otherwise. 2.The Contractor will provide on Demand Technical Phone Support, to Nuclear Medicine and Biomedical Engineering Staff as needed. The Contractor will provide all Upgrade Software as it becomes available to keep equipment performing within the manufacturer's specifications. All software upgrades must meet or exceed manufacturer's factory specifications to maintain compatibility with all Med View systems presently in place to include future performance and reliability upgrades. D. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. E. GOVERNMENT RESPONSIBILITIES The government will make the equipment available in accordance with the need and work with the vendor to resolve any difficulties. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1. Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size i.e. SDVOSB, VOSB, 8A, etc., and DUNS Number. Proof of Central Contractor Registration and Vet Biz registry (if applicable) is highly desirable. Identify all Joint Ventures and other Teaming arrangements. 2. A brief capability statement demonstrating past performance with relevant contract services in both Prime and or Sub-contractor role. 3.
