Description

PRESOLICITATION NOTICE The Great Lakes Acquisition Center (GLAC) has a requirement for one (1) acute care physician with services performed at the Tomah VAMC located in Tomah, WI. The contractor will provide all labor, materials, equipment and supervision necessary to perform acute care physician services in accordance with the statement of work, specifications, and terms and conditions listed within the Request for Quote ("RFQ"). The Government will award a single aggregate contract for these services and services are to be performed by a single individual. This will be a Labor Hour contract with a fixed hourly cost under North American Industry Classification System (NAICS) code 621111, size standard $10.0 million. This is a 100% Service Disabled Veteran-Owned Small Business (SDVOSB) set-side. The anticipated period of performance is Award though March 31, 2012, with two 3 month options. Electronic version will be made available on fbo.gov on or before December 20, 2011. The electronic version may be obtained free at the Federal Business Opportunities website: www.fedbizopps.gov, select "Find Business Opportunities" and then enter the solicitation number. Any amendments to this solicitation will also be available at this address. Offerors are advised that they are responsible for obtaining amendments.
