- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number A12PS00255 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. The complete text of any provisions and clauses may be accessed in full text at http;//farsite.hill.af.mil. The solicitation is a 100 percent set aside for small business under NAICS code 454312. The following commercial item is requested in this solicitation: Non-personal services for Delivery of liquefied petroleum gas to four Bureau of Indian Education (BIE) schools located on the Navajo Nation. Period of Performance is January 01, 2012 to December 31, 2012. A. Scope of Work. The contractor shall provide non-personal services for all labor, parts, materials, equipment, and transportation necessary to perform delivery of liquefied petroleum gas to four separate locations under the New Mexico Navajo North Agency. The four locations are located in Northern New Mexico and Northern Arizona. The contractor shall deliver the liquefied petroleum/natural gas supply in accordance with commercial standards, practices and local Navajo Nation laws and regulations. Liquefied petroleum/Natural gas supply shall be provided F.O.B. delivery point. B. Hours of Operation. Delivery of liquefied petroleum shall be performed between the hours of 8:30 a.m. and 3:30 p.m. Monday through Friday, excluding legal holidays. C. Place of Performance.1. Cove Day School, Red Valley, AZ; 2. Red Rock Day School, Red Valley, AZ; 3. Sanostee Day School, Newcomb NM and 4. Tiis Nazbas Community School, Teec Nos Pos, AZ. D. Quality Control. The BIE school representatives will routinely perform quality control checks during their scheduled deliveries. The government shall not provide facilities, equipment or materials to the contractor. The contractor shall provide, without cost to the government, all facilities, equipment and services required to deliver the liquefied petroleum/bulk propane. E. Quantity and Frequency. Delivery will be requested on an "As Needed" basis. The BIE school representative will notify the contractor when the level of propane has reached 40 percent or less of its rated capacity. The contractor will have 3 work days to deliver the liquefied petroleum gas. Quantity and frequency are based on usage by the students and staff at the four schools. Contractor shall utilize the below line item structure when preparing quotes: Line Item 0001Description: Delivery of liquefied petroleum gas/bulk propane to the Cove Day School.Non-personal services for Delivery of Bulk Propane to the school In Accordance With (IAW) the attached Statement of Work (SOW). Prospective contractor quotes should be based on a per gallon price of propane and all applicable taxes. Review the PWS for quantity and frequency for this requirement. Approximately 5,000 gallons. Cove Day School is located in Red Valley, AZ. Line Item 0002Description: Delivery of liquefied petroleum gas/bulk propane to the Red Rock Day School.Non-personal services for Delivery of Bulk Propane to the school In Accordance With (IAW) the attached Statement of Work (SOW). Prospective contractor quotes should be based on a per gallon price of propane and all applicable taxes. Review the PWS for quantity and frequency for this requirement. Approximately 8,000 gallons. Red Rock Day School is located in Red Valley, AZ. Line Item 0003Description: Delivery of liquefied petroleum gas/bulk propane to the Sanostee Day School.Non-personal services for Delivery of Bulk Propane to the school In Accordance With (IAW) the attached Statement of Work (SOW). Prospective contractor quotes should be based on a per gallon price of propane and all applicable taxes. Review the PWS for quantity and frequency for this requirement. Approximately 6,000 gallons. Sanostee Day School is located in Sanostee, NM. Line Item 0004Description: Delivery of liquefied petroleum gas/bulk propane to the Tiis Nazbas Community School.Non-personal services for Delivery of Bulk Propane to the school In Accordance With (IAW) the attached Statement of Work (SOW). Prospective contractor quotes should be based on a per gallon price of propane and all applicable taxes. Review the PWS for quantity and frequency for this requirement. Approximately 50,000 gallons. Tiis Nazbas Community School is located in TeecNosPos, AZ. Quotes can be emailed to marita.roth@bia.gov or faxed to 505-563-3019. Quotes are due by 3:00 p.m. MST on 12/23/2011.
