- SYNOPSIS DESCRIPTION: The Department of Veteran Affairs, Service Area Office (SAO) - East, Network Contracting Office (NCO) 8, 5201Raymond Street, Orlando, FL 32803, intends to solicit proposals under Request For Proposals (RFP) VA-248-11-RP-0874 for provision of eye examinations and eyeglass services to those veterans that require them and are eligible for assistance. The RFP is 100% set aside for SERVICE DISABLED VETERAN OWNED SMALL BUSINESSES (SDVOSB) Firms and shall be conducted in accordance with Federal Acquisition Regulations (FAR) Parts 12 and 15. All SDVOSB offerors submitting proposals must be registered and will be verified by the Contracting Officer in the VetBiz Information database located at www.vip.vetbiz.gov/. All offerors shall be registered on www.ccr.gov and https://orca.bpn.gov. NAICS Code 446130 applies to this solicitation. The awarded contract will contain a Base Year period with four one-year options. The Offeror shall possess all licenses, permits, accreditations and certificates as required by Florida law and shall furnish eye examinations and eyeglasses in strict accordance with all terms, conditions, provisions, specifications and schedules of this solicitation for Department of Veterans Affairs (VA) veteran beneficiaries at the corporate Orlando VA Medical Center. Facilities requiring onsite services: Orlando, FL VA Medical Center, 5201 Raymond Street, Orlando, FL 32803; and at Viera VA Outpatient Clinic, 2900 Veterans Way, Viera, FL, 32940. Facilities requiring offsite services: Orlando VA Medical Center, 5201 Raymond Street, Orlando, FL 32803; Viera Outpatient Clinic 2900 Veterans Way, Viera, FL 32940 and at Daytona Beach Outpatient Clinic, 551 National Health Care Drive, Daytona Beach, FL 32114. The solicitation reference number is VA-248-11-RP-0874. All information for this solicitation shall be posted on the Federal Business Opportunities (FBO) website at www.fbo.gov. The solicitation is scheduled to be uploaded to the website on or about December 29, 2011. No hard copies of the solicitation will be provided. Any amendments to this solicitation will be available at this website and Offerors are hereby advised that they are responsible for obtaining amendments to this solicitation at this website prior to the time of responding to the solicitation. Any questions pertaining to this solicitation must be provided to the Contracting Officer within seventy-two (72) hours after the posting of the solicitation, at either Alonzo.Jacobs@va.gov or the physical address listed below in this notice.
