- DESCRIPTION: This is a sources sought to determine the availability of potential sources having the skills and capabilities necessary to perform/provide the SPECIFIED REQUIREMENT. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. Questions should be submitted by email to iridious.ruise@va.gov. Provide only the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Service Disabled Veteran Owned Small Business (SDVOSB) relative to NAICS 541720 Research and Development in the Social Sciences and Humanities (size standard $7.0 million) Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on this website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. Responses must be emailed to iridious.ruise@va.gov no later than 1:00pm EST, 23 December 2011. Your response should include both the STATEMENT OF CAPABILITY as outlined in the Statement of Work, and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained below. Please place "Attention: Research and Development" in the subject line of your email. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: Submit a brief description [three (3) pages or more] that shows your company can provide specification as stated in the Statement of Work. Include past experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your availability to start date. Please indicate your address (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: Include the DUNS number of your firm. (h) State whether your firm is registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. PERFORMANCE BASED WORK STATEMENT 1. GENERAL: The Contractor shall provide qualified individuals to work on several quality improvement initiatives. These initiatives include the consult management project development, the Patient Aligned Care Team evaluation, development of a business case for certifications, and the staffing methodology implementation. An education affiliation with WJB Dorn VA with Dorn VA is preferred as well as previous work experience with the WJB Dorn VA Research Service Line. 2. BACKGROUND: The Research Service Line supports the Medical Center through developing unique evaluation and project implementation strategies. Consultations will be needed with Project Leads on system redesign projects and the collection of outcome data. 3. PERIOD OF PERFORMANCE: Date of award thru 7 Jan 2012. 4. PLACE OF PERFORMANCE: Bldg 9, room 218, Research Service, Columbia SC. 5. DAYS AND HOURS OF OPERATION: Monday - Friday, Intermittent hours between 8am - 4pm, as assigned, excluding federal holidays. 6. PERFORMANCE REQUIREMENTS: exactly what they will be doing and what are the outcomes -tasks and deliverables - report, develop program, etc. a. The Contractor shall provide qualified individuals understand the role of healthcare manager and are able to develop and complete a business plan for each project, process outcome data files, develop system redesign projects with clinical staff and other projects as designated. The deliverables will be a report for each project that details the baseline data, the quality improvement strategies and PDCA cycles used within the project, the interim data and analysis, and the final design of the project with a return on investment analysis. A written report and a storyboard will be presented for the Annual System Redesign Open House and for the formal program on Research Day. b. Preferred qualified individuals with a Health Administration background and an understanding of approaches to address the social and population health responsibilities of healthcare organizations. Credentials - workers who perform the work for this contract must have a baccalaureate degree and be enroll in a graduate program for health administration. Skills - These individuals must be able to manage data using Excel and Access programs, develop surveys, create a business plan, do a return on investment as part of a system redesign project, review the literature and provide an annotated bibliography identifying the level of evidence for the research, quality improvement or system redesign project. Additionally, these individuals must be able to demonstrate proficiency in writing reports and presenting data with graphs or charts. 7. CONTRACT PERFORMANCE MONITORING: The government has a requirement to monitor services in accordance with Performance Based Matrix. Tasks to be monitored include: baseline data acquisition; business plan development, return on investment analysis, meeting attendance and facilitation of teams, and development of final report. 8. INVOICES: Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer's Technical Representative (COTR), and submitted to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714. A properly prepared invoice will contain: oInvoice Number and Date oContractor's Name and Address oAccurate Purchase Order Number oSupply or Service provided oTotal amount due
