- Performance Work Statement (PWS) This combined synopsis/solicitation is being issued by 502CONS at Fort Sam Houston, Texas. RFQ No F2MRAH1284AQ01 applies and incorporates provisions and clauses in effect through Federal Acquisition Regulations. The proposed acquisition will require the contractor to provide maintenance to fitness center equipment at four locations located at Fort Sam Houston and Camp Bullis for twelve months. See the attached Performance Work Statement (PWS) for additional requirements. It is the vendor's responsibility to monitor the solicitation for amendments issued in reference to this requirement. All Contractors who provide goods/services to the DOD must be registered in the Central Contractor Register (CCR). Online Certifications and Representations is also required http://orca.bpn.gov. Failure to comply with ORCA registration may deem offeror ineligible for award. Submission of Offers: Submit offers to the office specified in this synopsis/solicitation at or before the exact time specified in this synopsis/solicitation. As a minimum, offers must show: (1) The solicitation number; (2) DUNS and CAGE number (3) The name, address, e-mail and telephone number of the offeror; (4) Terms of any express warranty; (5) Price and any discount terms; (6) Payment Terms (7) Include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the synopsis/solicitation. Offers that fail to furnish required representations or information or reject the terms and conditions of the synopsis/solicitation may be excluded from consideration.
