Description

Introduction This is a small business Sources Sought Notice. This is NOT a solicitation for proposals or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses, HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your response to this information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The North American Industry Classification System (NAICS) code is 423450 with a business size standard of 100 employees. An organization that is not considered a small business under NAICS code 423450 should not submit a response to this notice. General Requirements The Department of Veterans Affairs (VA), VA is seeking potential small businesses to supply hemo-dialysis supplies. The hemo-dialysis supplies is to be provided for the Memphis VA Medical center, located at 1030 Jefferson Avenue, Memphis, TN 38104, during a five (5) year specified period (one base year, plus four option years). Brand name or equal descriptions will be included in the solicitation - products must be compatible with the manufacturer make and model number of the dialysis equipment: i.e. Gambro Phoenix Kidney Machine and the Next Generation of Cobe Kidney Machines. This is not a request for competitive quotes. Responses to this sources sought notice may include general information regarding your company name, address, contact person, phone number, email address, type of business, such as 8(a), small business, hub zone, small disadvantaged business, service disabled Veteran owned or Veteran owned. Information regarding your firm's capacity and capability to provide such supplies are to be submitted on or before December 29, 2011 prior to 12:00 pm (EST) addressed to alanna.crawford@va.gov. Telephone responses are not permitted. No solicitation document is available at this time. Qualified vendors may be required to respond to a separate solicitation announcement. Interested small business organizations are required to identify their size (i.e., 8(a), HUBZONE, WOSB, Veteran Owned, Small Disadvantaged Business, Service Disabled Veteran Owned, etc.), in accordance with the Small Business Administration. Please note, all SDVOSB and VOSB must be verified by the office of Small Disadvantaged Business Utilization Center for Veterans Enterprise (CVE) prior to receiving any award. The Government requests that no proprietary or confidential business data be submitted in a response to this notice. However, responses that indicate the information therein is proprietary will be properly safeguarded for Government use only. Capability statements must include the name, email address, and telephone numbers of a point of contact having authority and knowledge to discuss responses with Government representatives. Capability statements in response to this market survey that do not provide sufficient information for evaluation will be considered non-responsive. All capability statements sent in response to this Sources Sought Notice must be submitted electronically (via email) to Alanna Crawford, Contract Specialist, at alanna.crawford@va.gov in either MS Word or Adobe Portable Document Format (PDF) by December 29, 2011 at 12:00 p.m. EST, in order to be considered. Do not contact medical center staff in regards to this requirement, as they are not authorized to discuss matters related to this procurement action. All questions should be submitted in writing to the Contract Specialist, Alanna Crawford at alanna.crawford@va.gov. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. If a solicitation is issued, it will be announced and published in Federal Business Opportunities, and interested parties must respond to the solicitation to be considered. Responses to this notice will not be considered adequate responses to a solicitation.
