Description
Introduction This is a small business Sources Sought Notice. This is NOT a solicitation for proposals or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses, HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your response to this information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The North American Industry Classification System (NAICS) code is 339113 with a business size standard of 500 employees. An organization that is not considered a small business under NAICS code 339113 should not submit a response to this notice. General Requirements The Department of Veterans Affairs (VA), VA Integrated Service Network VISN 9, is seeking potential small businesses to supply orthotics and artificial limbs and related services/repairs. The artificial limbs and orthotics and related services are to be provided to the following facilities located at: 1310 14th Avenue South, Nashville, TN, 3400 Lebanon Pike, Murfreesboro, TN, 1030 Jefferson Avenue, Memphis, TN, 1101 Veterans Drive Lexington, KY, 800 Zorn Avenue, Louisville, KY, PO Box 4000/Sydney &Lamont Street Building 205 Mountain Home, TN and 1540 Spring Valley Drive Huntington, WV, VA Medical centers during a five (5) year specified period (one base year, plus four option years). The contractor must be located within a 60 mile radius of at least one of the VA Medical Centers listed above. Interested small business concerns must be American Board Certified (ABC) or Board for Orthotist-Prosthetist Certified (BOC).The work to be performed must comply with the Joint Commission standards, however certification and/or accreditation by the Joint Commission is not required. A copy of these standards may be obtained from the Joint Commission, One Renaissance Blvd., Oakbrook Terrace, IL 60181. Capability Statement Small Business concerns that believe they possess the capabilities necessary to undertake this requirement should submit complete documentation of their capabilities. Documentation of technical expertise and capability must be presented in sufficient detail for the Government to determine that your organization possesses the necessary functional area expertise and experience to compete for this acquisition. The capability statement should include 1) the total number of employees, 2) documentation showing that the organization is an accredited facility by ABC or BOC, 3) a description of facilities, including geographic location and mileage from the specified VA facilities, 4) a description of similar contracts awarded and/or currently being performed, and 5) Professional qualifications of individuals providing supplies and services to VA Beneficiaries. The capability statement must not exceed six (6) single sided or three (3) double sided pages in length, and using a 12-point font size minimum. Interested small business organizations are required to identify their size (i.e., 8(a), HUBZONE, WOSB, Veteran Owned, Small Disadvantaged Business, Service Disabled Veteran Owned, etc.), in accordance with the Small Business Administration. Please note, all SDVOSB and VOSB must be verified by the office of Small Disadvantaged Business Utilization Center for Veterans Enterprise (CVE) prior to receiving any award. The Government requests that no proprietary or confidential business data be submitted in a response to this notice. However, responses that indicate the information therein is proprietary will be properly safeguarded for Government use only. Capability statements must include the name, email address, and telephone numbers of a point of contact having authority and knowledge to discuss responses with Government representatives. Capability statements in response to this market survey that do not provide sufficient information for evaluation will be considered non-responsive. All capability statements sent in response to this Sources Sought Notice must be submitted electronically (via email) to Alanna Crawford, Contract Specialist, at alanna.crawford@va.gov in either MS Word or Adobe Portable Document Format (PDF) by December 29, 2011 at 12:00 p.m. EST, in order to be considered. Do not contact medical center staff in regards to this requirement, as they are not authorized to discuss matters related to this procurement action. All questions should be submitted in writing to the Contract Specialist, Alanna Crawford at alanna.crawford@va.gov. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. If a solicitation is issued, it will be announced and published in Federal Business Opportunities, and interested parties must respond to the solicitation to be considered. Responses to this notice will not be considered adequate responses to a solicitation.
