- RFQ VA24812Q0455 QUESTION & ANSWERS Question Mr. Jones: Please clarify what type of pricing you would like on Item Nos: 0002, 1002, 2002, 3002, and 4002 (VA24812Q0455 - Dialysis water treatment system) It looks as though you are asking for a monthly price. However, under the description, the solicitation states "Contractor shall provide written estimate prior to repairs." Please call or email to clarify. Thank you and have a great day! Answer: Item Nos: 0002, 1002, 2002, 3002, and 4002. Contractors are not required to submit information on the place holder line items. The line items are funding place holders for emergency service and repairs. If emergency repairs are conducted outside of the maintenance agreement; funding will be provide on the place holders line item for payment. Emergency repairs shall have a written estimate and be approved prior to commencement of service. Contractors shall provide emergency/after hours /overtime labor rates and travel rate for emergency calls (if applicable) in an addendum page.
