- Attachment 05 Abbreviations Attachment 04 HHS Subcontracting Plan Form Attachment 03 Disclosure of Lobbying Attachment 02 Title 45-Part 5B Attachment 01(a) Supplies/Services RFP with all sections, EXCEPT Section B: Supplies/Services SF 1449 - RFP NIHES2012136 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. The solicitation number is NIHES2012136 and is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-54. The Contracting Officer is contemplating issuing a Firm Fixed Price (FFP) type contract with Labor Hour (LH) and Time & Material (T&M) line items as required. The associated North American Industrial Classification System (NAICS) Code for this procurement is 541380 which has a Small Business Size Standard of $12.0 million dollars. The estimated period of performance for this requirement is one base year and four 12 month option years. Interested parties shall submit their proposal to Mr. Stephen L. Cannon, Superviory Contracting Officer by email @ cannons@niehs.nih.gov, Phone: (919) 541-0423. Proposals are due 6 Jan 2012 NLT 4:30pm local estern time, submitted via email to Stephen L. Cannon and must include the solicitation number in the subject line. Each question submitted shall reference the specific section and paragraph of the RFP as appropriate. Proposals must be received by the Contracting Officer no later than the time specified in the solicitation to be considered for award.
