Description
Applicable Provisions & Clauses. FAR 52.222-48 is included for submission with your quote. The Uniformed Services University of the Health Sciences (USUHS) located on the Walter Reed National Military Medical Center in Bethesda, MD has a requirement for an annual full service and maintenance agreement on the four (4) Kodak Film Processors identified below, for a base period of twelve (12) months with two (2) twelve (12) month options: • Kodak M35A Film Processor, Serial No. 12854 • Kodak 2000A Film Processor, Serial No. 215593 • Kodak M35A Film Processor, Serial No. 40533 • Kodak 2000 Film Processor, Serial No. 412042 (Additional systems may be added over the life of this resulting option year purchase order at the unit cost identified in the active period.) The Contractor is to provide monthly preventive maintenance (12/year) and emergency repairs on an as-needed basis on the equipment identified below, to include chemistry developer and fixer quantities. Contractor is to provide all labor, chemistry filters, materials, tools, transportation and management necessary to maintain and repair the equipment identified below on an as-needed basis in strict accordance with OEM service and performance specifications. Major assembly parts such as racks, motors, pumps, drive train are excluded. Parts, if needed, will be paid with a GPC. The anticipated period of performance for the base period is from 1 February 2012 through 31 January 2013. This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This request for quotes (RFQ) for commercial items is in accordance with FAR Part 12 - Acquisition of Commercial Items and FAR Part 13 - Simplified Acquisition Procedures. It will be the responsibility of the offeror to check the website http://www.fbo.gov for the issuance of any amendments to the solicitation. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-54 (effective 2 December 2011). This solicitation is set-aside for small businesses under the applicable North American Industry Classification System (NAICS) Code 811219 - Other Electronic Precision Equipment Repair. All responsible sources may submit a quote, which will be considered by USUHS. See attached provisions and clauses. Offerors must complete FAR 52.222-48 (page 2 of the Provisions & Clauses document. To be eligible for award, offerors must register, or already be registered with the Central Contractor Registration (CCR) database and the Online Representations and Certifications Application (ORCA). For more information, please see https://www.bpn.gov/ccr/default.aspx and https://orca.bpn.gov/login.aspx. Quotes are due on or before 7:00pm EST on Monday, 9 January 2012. Quotes shall be submitted via email to the Contract Specialist, Melissa T. Marcellus, at melissa.marcellus@usuhs.mil. Quotes received after this 9 January 2012 deadline, or via any means other than email will not be evaluated. All questions/clarifications in reference to this RFQ must be submitted via email to the above email address ( melissa.marcellus@usuhs.mil ) no later than 12:30pm EST on Wednesday, 4 January 2012. Only electronic submissions of questions/clarifications will be accepted. Questions/clarifications will be addressed in an amendment to the solicitation. Any questions/clarifications submitted after 12:30pm EST on 4 January 2011, or via any means other than what is specified above, will not be answered. The Government will award a single firm fixed price (FFP) option year purchase order with a base period of 12-months and two (2) 12-month options, resulting from this solicitation to the responsible offeror whose quote, conforming to this notice, will be most advantageous to the Government. Award will be made to the responsible offeror representing the best value to the Government using tradeoffs between the evaluation factors identified below. The Government is not required to award to the lowest priced offeror, or the highest technically-rated offeror, or the offeror with the strongest past performance. Quotes received by the deadline of 7:00pm EST on Monday 9 January 2012, will be evaluated using the following evaluation factors (listed in order of importance): 1. Price - In order to be considered for award, pricing must be submitted for the base period and both option periods. Include all applicable costs (i.e., fuel surcharges). 2. Technical - Offerors are to provide a maintenance schedule for the equipment identified above. Offerors responding to this requirement must provide a detailed maintenance plan which includes, but is not limited to, emergency repairs, telephone support, replacement of parts, support availability, response time, PM visits, etc. 3. Past Performance - The Government shall evaluate the offeror's present and past performance on similar tasks within the past three years. Offerors must provide two (2) customer references, for similar work performed. The Government reserves the right to select which reference(s) to contact. The following must be provided for each listed contract: • Name of client and address • Name of current point of contact including telephone number, e-mail address and title • Contract number • Contract value • Contract period of performance • A clear description of the supplies/services provided USUHS may request additional information at any time. Offerors will have an opportunity to address any adverse past performance history. Offerors who do not have any past performance history will not be rated favorably or unfavorably, but will instead receive a Neutral/Unknown rating for past performance. Offerors must clearly state in their quote if they do not have at least two (2) past performance references. Failure to provide this information may render an offeror's quote non-responsive and may not be considered for award. A strong history of relevant past performance may be more advantageous to the Government than a less relevant record of favorable performance. The Government reserves the right to use data provided by the offeror and data provided by alternative sources in conducting its assessment of past performance history. The Government reserves the right to award without discussions. Each offeror is urged to examine the solicitation and ensure that the submitted quote contains all necessary information, provides all required documentation, and is complete in all aspects.
