PERFORMANCE BASED STATEMENT OF WORK (SOW) Prepared by PHYSICAL THERAPY AT FORT BELVOIR COMMUNITY HOSPITAL (FBCH) 16 DECEMBER 2011 1.0 INTRODUCTION 2.0 SCOPE 3.0 SCHEDULE 4.0 CONTRACTOR PROFESSIONAL SERVICES AND RESPONSIBILITIES 5.0 EQUIPMENT OWNERSHIP 6.0 CHANGE CONTROL PROCEDURES 7.0 PLACE OF PERFORMANCE 8.0 HOURS OF WORK 9.0 QUALITY CONTROL 10.0 POINTS OF CONTACT 11.0 ADDITIONAL REQUIREMENTS 12.0 BASE ACCESS REQUIREMENTS 1.0INTRODUCTION This Statement of Work (SOW) is for a Contractor to perform maintenance service for Physical Therapy Pool at Fort Belvoir Community Hospital. 2.0SCOPE The Contractor shall furnish all labor, material and equipment to provide complete maintenance service up to twice a week. All chemical and maintenance services shall be performed in accordance with manufacturer's standard commercial practices, TB 575 Occupational and Environmental Health, Industrial Safety. A.Perform pump and filter manufacturer's recommended maintenance B.Vacuum/clean pool C.Operation checks D.Backwashing E.Water chemical treatment F.Chlorine and Carbon Dioxide G.Log all visits 3.0 Schedule The Contractor shall service the pool to maintain good operating conditions two times a week. The days of service are Sunday and Wednesday. Normal operating hours are 0730 a.m-1630 p.m. the Contractor shall report in person or telephonically to notify the Physical Therapy front desk clerk (571) 231 2198/9, of their arrival. The Contractor will sign in and be escorted by a Physical Therapy Staff Member or Hospital Security to the pool area prior to commencing service. After normal operating hours, the Contractor shall report to the Staff Duty of the day (571) 231 3068 located on the 1st floor of The Oaks Building. The Contractor will be escorted by Hospital Security (571) 231 3821, to the pool prior to commencing service. The Contractor will provide a list with the names of the Pool Technicians authorized to enter the pool area of Fort Belvoir Community Hospital. Hospital Security will only permit authorized personnel in to the pool area in the event a Physical Therapy Staff member is not present. 4.0 Contractor Professional Service Responsibilities The Contractor shall be certified pool operators to manage pool chemistry, testing, treatment to OSHA and installation regulations. The Contractor shall present a neat and clean appearance. All outer clothing shall be laundered or dry cleaned as or dry cleaned as appropriate, well fitting and fresh each workday. Contractor shall wear pants or slacks, shirt with collar sleeves, appropriate (preferably non-slip) shoes and socks. Tank tops, cut-off pants, shower shoes or similar items of apparel are specifically prohibited. Hair to include facial hair shall be nearly groomed. 4.1 Manufacturer's recommended maintenance The Contractor shall perform all manufacturers' recommended maintenance to the pool's sanitizer system, chemical controller, and sand filter. The models currently in use are: Del Genesis Ozone Sanitation System Triton II Commercial Sand Filter Polaris Watermatic The Contractor must immediately notify the FBCH Physical Therapy of any failures, breakage or problems of installed equipment, components, or piping. The Contractor shall perform no pool maintenance work outside of this SOW. Any additional work needed to maintain the FBCH Physical Therapy Pool over and above this SOW must be negotiated, approved by Fort Belvoir Community Hospital and paid for separately from this contract. 4.3 Initial and weekly pool maintenance The Contractor shall provide all the materials necessary to perform an initial pool cleaning and initial pool water treatment at the beginning of the contract. The initial cleaning and water treatment must return the pool back to a safe, clean, clear state and achieve the water test parameters. The Contractor will provide one annual cleaning of the pool, drain, rails, gutters, power wash the deck and clean the caulking to maintain optimum function of the sanitization and filtration systems. After the acceptance of the initial cleaning and water treatment, the Contractor shall provide all the materials necessary to clean the pool and chemically treat the pool water twice a week for the duration of the contract. The Contractor will respond to emergencies (ie chemical imbalance, biological material in the pool) within 4 hours of notification. Emergency responses are not included or part of the bi-weekly maintenance requirements. The Contractor will maintain liability property damage insurance policy. FBCH will not be liable for loss, injury, and or damage sustained as a result of the use of the pool or facilities, excepting injury, damage or loss caused by negligence, of the Contractor and contracting staff. The Contractor is also liable for damage that results from defective workmanship, and maintenance on the part of the Contractor and their employees. 4.4 Vacuum/clean pool The Contractor shall, twice a week, vacuum the pool floor, skim the pool surface, and clean out the three (3) skimmer baskets. 4.5 Operational checks The Contractor shall perform operational checks to ensure the pump and pool circulation is performing adequately, all valves are in good working order, the pool drain line is functioning properly, and all modes of the sand filter, ozone sanitation system and chemical controller are functioning properly. 4.6 Backwashing The Contractor shall backwash the filter twice a week to ensure good operational order of the pool. 4.7 Water chemical treatment The contactor shall perform all chemical tests to include: Chlorine residual, pH, Alkalinity, Calcium, and Stabilizer level. The Contractor shall add chemicals when needed. The Contractor shall provide all chemicals needed for the tests and added to maintain the levels. The pool chemistry shall be within the following limits: Check complete pool chemistry. a.Bacteriological: HCP not to exceed 200 CFU per 1.0 ml b.PH: 7.2-7.8 c.Free available chlorine: 2-3.5 ppm (mg/L) d.Max chlorine>4ppm Balance pool chemistry as needed. Check on all skimmers, and filtration equipment. Record all chemical levels and filter adjustments. 4.8 Liquid Chlorine and Carbon Dioxide The Contractor shall furnish all supplies (to included but not limited to Liquid Chlorine and Carbon Dioxide) to comply with the requirements of this contract, except where specifically identified as government-furnished supplies and services 4.9 Log all visits The Contractor shall log all visits with date/time, technician's name, maintenance performed, and any other notations necessary. The log paperwork will be kept at the Physical Therapy front desk. 5.0 Equipment ownership The Contractor shall purchase all chemicals and equipment needed to perform this job. The equipment shall be owned by the Contractor and the Contractor shall be responsible for the maintenance of the equipment. No equipment will be stored at Fort Belvoir Community Hospital. 6.0Change control procedures The Government may request changes in or additions to the work being provided here under by written modification to the Contractor by a warranted contracting officer. Contractor will comply, to the extent feasible, with such requested changes. Should the Contractor determine that such changes cause an increase or decrease in the cost of or time required for performance of the work, Contractor shall advise the COR thereof. The customer requested changes shall not become effective, until agreed upon and executed by Contractor and contract officer. Contractor will begin work in response to Government's requested changes upon receipt of the written notification, a Purchase Order, and Addendum to the SOW for additional work. 7.0 Place of Performance The location of installation is 9300 Dewitt Loop Fort Belvoir Community Hospital. The Contractor is expected to work twice a week excluding federal holidays. Contract shall begin 15Jan 2012 to 14 Jan 2013. 8.0 Hours of Work A notice of a request for scheduled absence shall be provided prior to the date of the planned absence. It is the responsibility of the Contractor company to provide personnel to ensure the maintenance of the therapeutic pool. Unscheduled absences due to illness, emergency, or are an inability to work during a scheduled shift, must be called into the Contractor Company. It is the responsibility of the Contractor Company to provide personnel to ensure maintenance of the therapeutic pool. During anticipated closure of the facility due to Command declared training holidays, administrative leave granted to the entire staff, or other unplanned closure of the facility due to natural disasters military emergency, or severe weather, Contractors who are scheduled to work, shall report to their place of duty unless notified differently. The following is a list of legal federal holidays as referred to elsewhere in the contract/task order. Contractors may be required to work on legal holidays as determined by the department chief. In the event pool maintenance is scheduled on a holiday. New Year's Day, January 1st Martin Luther King's Birthday, 3rd Monday in January President's Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4th Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veteran's Day, November 11th Thanksgiving Day, 4th Thursday in November Christmas Day, December 25th Should circumstances beyond the control of the Fort Belvoir Community Hospital, such as acts of god, inclement weather, power outages or temporary closure of facilities, bar or hinder the Contractor to perform. The Contractor contact the NCOIC of the Physical Therapy Pool or front desk 571- 231- 2198 to reschedule. 9.0Quality Control Upon the request of the COR the Contractor shall participate in any Government Quality Assurance inspection. Upon completion of service by the Contractor's service representative, an initial written service report shall be provided to the customer and COR providing detailed information regarding the service provided to meet OSHA and Installation safety prevention regulations. Such regulations include chemical safety and waste disposal. 10.0 Point of Contact (POC) Points of contacts shall be provided at the time of award of the contract. 11.0 Additional Requirements The work area shall be kept as clean as possible during working hours. Debris will not be allowed to accumulate on the work site. All debris will be removed from the work site by the Contractor and deposited in an authorized container. 12.0 Base Access Requirements 12.1 In order to Access FBCH the Contractor must provide a valid driver's license and valid vehicle registration. 12.2 Contractors are divided into two categories, to ensure proper screening and to minimize installation access delays, as follows: 12.2.1 Category 1 (Cat-1) - Primary and Subcontractors who are identified against a contract prior to the contract performance start date. 12.2.2 Category 1 (Cat-2) - Subcontractors and affiliate workers not previously identified against a contract prior to contract performance start date. 12.3 Contractors are required to provide an EAL (entry authorization list) all Cat-1 employees which must include the following information: 1.Contract Number 2.Work Site/Location 3.Inclusive dates of the contract (start date - end date) 4.Employee Name (Last, First, MI) 5.Date of Birth 6.Social Security Number 12.4 Contractor is to have each employee identified on the EAL complete FBCH Form 251, Contractor Consent to Background Check and submit it with the request for issuance of FBCH Contractor Badge. 12.5 Contractor shall be notified of any individuals whose background check meets any of the following 12 disqualifiers as they will not be allowed access to the installation. 12.5.1 U.S. citizenship, immigration status, or Social Security Account Number cannot be verified. 12.5.2 Barred from entry/access to any military installation or facility. 12.5.3 Individual is wanted by federal or civil law enforcement authorities, regardless of offense or violation. 12.5.4 Individual's name appears on any federal agency's list for criminal behavior or terrorist activity. 12.5.5 Individual was convicted of espionage, sabotage, treason or terrorism. 12.5.6 Individual was incarcerated for 12 months or longer within the past three years, regardless of offense or violation. 12.5.7 Individual was convicted of a firearms or explosives violation within the past three years. 12.5.8 Individual has been convicted of illegal possession or use of drugs/narcotics on more than one occasion within five years from the date access to FBCH is requested. 12.5.9 Individual has been convicted of an offense involving drug trafficking, possession with intent to sell or drug distribution within 10 years from the date access to FBCH is requested. 12.5.10 Individual has been convicted of a felony involving violence against a person, arson, robbery or burglary within 5 years from the date access to FBCH is requested. 12.5.11 Individual is required to register as a sex offender under federal law or the applicable state law. 12.5.12 Individual has been convicted of any crime involving indecent acts with a minor or a felony that is sexual in nature. 12.8 Individuals adjudicated and cleared under the disqualifying criteria are considered cleared for installation access for a two-year period. Thereafter, renewal is required. 12.9 For complete information regarding Visitor Entry request a copy of the AFI 31-101 from your COR.
