- An updated copy of Attachment IV. USE THIS EXCEL VERSION. A copy of Amendment 0001. This amendment needs to be signed and submitted with your proposal. The purpose of this amendment is to: 1) extend the response date and proposals; and 2) update Attachment IV, Pricing Spreadsheet. 1. All questions must be submitted in writing via e-mail to contractingdivision@usuhs.mil by January 4, 2012 referencing the solicitation number. 2. The response date for proposals are due on February 6, 2012 at 10:00 a.m. 3. Contractors are required to utilize Attachment IV for their supportive pricing documentation.
