- Clarification to previous notice for VA528-12-R-0047: REMODEL WARD 9C: This requirement is set aside for small business. If you are a small business and you qualify for one of the small business programs outlined below, evaluation preference points will be provided. Preference points will be provided in descending order importance (no. 1 will receive the highest amount of points, etc): 1. Service Disabled Veteran Owned Small Business (SDVOSB) 2.Veteran Owned Small Business (VOSB) 3.Women Owned Small Business (WOSB) All SDVOSB and VOSB, at the time of submission of SF 330, must represent to the contracting officer that it is a- (1) SDVOSB concern or VOSB concern; (2) Small business concern under the North American Industry Classification System (NAICS) code assigned to the acquisition; and (3) Verified for eligibility in the VIP database - http://vip.vetbiz.gov One Standard Form 330 shall be prepared for the entire team. Example projects which best illustrate proposed team's qualification for this contract shall be limited to ten (10) projects for the entire team. Mail or hand deliver three copies of SF 330 to Rosanne J. Raczkowski - (90NCA), VA Western New York Healthcare System, 3495 Bailey Avenue, Buffalo, NY 14215 (Room No. is 67 Wing C (Basement). All responses are to be received by the Contracting Officer no later than 3:30pm on January 4, 2012. Ward 9C is an existing space to be remodeled. There are no site visits scheduled at this time.
