Description
The Department of Veterans Affairs 646 is considering whether or not to set aside an acquisition for HUB Zone concerns or for Service-Disabled Veteran-owned Small Business concerns (SDVOSB), Veteran Owned Small Business (VOSB), Woman Owned Small Business (WOSB) or for Small Businesses. The NAICS code is 238290 and the small business size standard is less than $14 mil. All of the above must be submitted in sufficient detail for a decision to be made on a HUB Zone concerns or for Service-Disabled Veteran-owned Small Business concerns (SDVOSB), Veteran Owned Small Business (VOSB), Woman Owned Small Business (WOSB) or for Small Businesses set-aside. Failure to submit all information requested may result in an unrestricted acquisition. A decision on whether this will be pursued as a HUB Zone concerns or for Service-Disabled Veteran-owned Small Business concerns (SDVOSB), Veteran Owned Small Business (VOSB), and Woman Owned Small Business (WOSB) or for Small Businesses, will be posted in the FedBizOpps website at http://www.fedbizopps.gov. 1 Annual maintenance contract for 12 MO maintenance, repair and quarterly PM inspection of all automatic door operator equipment and located throughout the VA Pittsburgh Healthcare System as listed below for the period of Date of Award for One year with a base and four option years: HIGHLAND DRIVE DIVISION 7180 Highland Drive Pittsburgh, PA 15206 OAKLAND DIVISION University Drive "C" Pittsburgh, PA 15240 H J HEINZ III DIVISION 1010 Delafield Road Pittsburgh, PA 15215 QUALIFICATIONS: Contractor must be a qualified automatic door contractor who performs maintenance on electrically operated automatic doors on a full-time basis, 40 hours per week. Work must be performed by qualified service personnel, following the manufacturer's recommendations or any procedures deemed necessary for the optimum performance of the equipment. CONDITIONS OF WORK: Door service will be provided under this agreement during normal business hours of 7:00am- 3:30pm, Monday through Friday, excluding holidays. Any calls handled other than the above-stated time, will require issuance of a separate purchase order to include cost for labor only. Emergency repairs must be completed within twenty-four hours. Non-emergency repairs must be completed within forty-eight hours. The contractor shall submit an invoice for emergency work performed outside normal duty hours of 7:00a- 3:30pm, Monday through Friday, indicating time required for repairs. TECHNICIAN MUST DOCUMENT TIME IN AND TIME OUT ON JOB TICKET FOR WORK PERFORMED AFTER NORMAL WORKING HOURS. Materials used on all repairs shall be considered part of this contract and may not be included in the emergency invoice. Requests for service after normal duty hours may be initiated only by the Contracting Officer or his/her designee who will be designated after contract award. Contractor must provide a contact phone number(s) for emergency service outside of normal duty hours. A list of contact personnel and phone number for service calls twenty-four hours per day, seven days a week is to be provided in writing, upon contract award. All technicians will display photo identification when required to enter VA property and then report to the VA Police Desk to sign in, exchange driver's license for VA Access Control Identification badge, and after work is complete, return to sign out, exchange ID badge for their driver's license and leave a copy of the service report. The VA Police are located as listed below: VAMC Highland Drive VA Police Desk- Bldg 1 ambulance entrance VAMC Oakland VA Police- Bldg 1 entrance VAMC Aspinwall VA Police Desk- Bldg 51, Rm 1A102 Contractor shall advise the VA Police of status of the doors before leaving. SCOPE OF WORK: Maintenance, repair or replacement of all components of the listed electric automatic doors as required maintaining the electric doors in first class operating condition. Quarterly preventive maintenance inspections of all doors including any adjustment, lubrication or alignment. A written report listing the doors inspected and corrective work will be provided within 30 days after completion of these inspections to the appropriate shop foreman as listed: VAMC Highland Drive Division COTR VAMC Oakland Division COTR VAMC Aspinwall Division COTR All inspections will be scheduled by contacting the above listed foreman as appropriate. All repair work, preventive maintenance inspections or alterations to existing doors will be performed in accordance with the door manufacture- er's specifications, and the National Electrical Code. All cost of material, equipment, and labor will be included in the proposed contract bid except for the following: Exceptions: Door frames, glass and damage by neglect are not included in the scope of the work. Service required due to damage caused by accident, fire, acts of God, acts of vandalism, riot, strike, floods, earthquakes, lightning or storm, is not covered by this agreement. The contractor is responsible for providing professional work by qualified personnel. Improper work, unfinished work, or failure to commence work within the specified time limits may result in adverse contract action. The contractor is responsible for stocking commonly used material, parts, equipment, etc. necessary for repair work within the prescribed timeframe as required. It is the VA's responsibility to inspect daily the doors according to procedures in the distributor's Daily Check form. If equipment is malfunctioning or an unsafe condition exists, the VA shall immediately turn off the door operating equipment and notify the contractor. Emergency service performed after normal business hours and after issuance of a separate purchase order, will be billed at a charge of $__________/ per hour to cover premium labor rates. The rate quoted will not be a factor in contract award.
