Description
STATEMENT OF WORK TEMPLATE A.GENERAL INFORMATION 1. Title of Project: Event Services 2. Scope of Work: The VISN 3 MIRECC will establish an account with the Contractor for FY 2012 in the amount of $21600. The Contractor will provide the following services: 1.Study participant reimbursement for time and travel: $8,400 (approximately) 2.Event Planning (1 meeting per year) includes rental fees, AV, breaks: $3200 (approximately-based on event held in FT 2011) 3.Reimbursements to Staff for professional expenses for scientific meetings including poster preparation, reprints, etc. $3000 approximately) 4.Per diem and travel reimbursements and hotel for external advisors for annual external advisory board meeting: $4800 approx (4 advisors @$1200) 5.Refreshments for veteran/family focus groups held throughout FY ($1000 approx) 3. Performance Period: The contractor shall complete the work required under this SOW in _365 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. [Time and materials] The work shall begin within 5 calendar days of award, unless otherwise specified. [Both] Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. 5.
