- Draft Performance Work Statement This sources sought notice is issued for information and planning purposes only. This announcement is not a Request For Proposal (RFP) nor does it commit the Government to solicit or award a contract now or in the future. No solicitation is available at this time. The purpose of this notice is to identify sources capable of meeting the needs of the Government as stated herein. All businesses large and small, 8(a), SDB, WOSB, HUBZone, SDVOSB, etc., capable of performing the requirement indicated in the attached Performance Work Statement are encouraged to respond to this notice. Luke Air Force Base, Arizona has a requirement for one Flight Medicine Provider to provide medical services for the 56th Medical Group. Luke anticipates using FAR Part 15 -- Contracting By Negotiation to award a Firm Fixed Price contract to one offeror. The draft Performance Work Statement is attached which details the requirements and qualifications Luke AFB anticipates will be included in the contract. Sources that have the ability to provide the necessary support services are asked to respond to this notice with a brief capability statement (maximum five pages) to provide a clear and complete narrative of applicable capabilities to meet the requirements. Responses should include:company name and address, DUNS number, CAGE code, size of business as it relates to the Primary NAICS code 621999, information on any additional small business classifications granted (if applicable), current point of contact (name, phone number, and email), and a list of contracts within the past four (4) years under which the interested party has performed the same or similar work. All responses must be emailed to Lynita.Hinsch@Luke.af.mil prior to 5:00 pm MST Friday, 13 January 2012.
