- STATEMENT OF WORK TEMPLATE A.GENERAL INFORMATION 1. Title of Project: [1 year Lease Proposal and Maintenance Agreement for East Orange, Lyons, Brick and East Orange Pharmacy Veterans Affairs Medical Center locations] 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The following Maintenance must be included in the agreement: all labor, all parts, insurance against downtime and inconvenience, budgeting assistance by combining the maintenance costs with the annual maintenance agreement and/or a guarantee option covering several years, unlimited toll-free customer support for basic questions and concerns, fastest response time possible, on-site service response target of 8 business hours, remote diagnostics to maximize uptime by predicting potential failures, 100% Complete Satisfaction Guarantee, service history maintained electronically for faster system diagnosis, full quality checks with each service call, preventative maintenance, and use of only factory certified parts. Background: Lease Proposal for equipment stated above for 1 year. Lease contract to run 10/01/2011-09/30/2012. 4. Performance Period: [Firm-Fixed-Price] The contractor shall complete the work required under this SOW in _30__ calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. 10/01/2011-09/30/2012. 5. Type of Contract: [Firm-Fixed-Price or Time and Materials].
