The purpose of this notice is to obtain information regarding: General Statement of Need: The availability and capability of qualified business sources that can provide surgical supplies as listed herein, as requested by authorized VA Staff, in order to support the West Roxbury campus of VA Boston Healthcare System (VABHS), 1400 VFW Parkway, West Roxbury, MA, 02132. All disposable supplies provided under this contractor shall be F.O.B. Destination; fuel surcharges shall not be allowed nor added to the invoice. The VABHS is seeking one responsible vendor that can provide all of the supplies listed below. LINE 1-3, 5, 8: Intuitive drapes are needed for use with the Da Vinci robot already on station. Line 4, 6: Bladeless Obturators 8mm, used in conjunction with Intuitive Seal stk#400077 Single use device for use in thoracic, abdominal, and gynecologic invasive surgical procedures. Provide pathway for the introduction of endoscopic surgical devices. Line 7: 400077 Cannula seal: Single use seal Converts to 5mm Applications in endoscopic surgery for establishment of a port of entry for endoscopic instruments. Use in conjunction with Intuitive surgical cannula in order to maintain pneumoperitoneum. Consists of an internal valve and seal to prevent gas leakage when instruments are inserted or withdrawn 10mm/12mm seal includes a built-in 5mm converter to prevent leakage and allow insertion of instruments of different diameters than the main seal. Not to be used with instruments/cannula that have outside diameters less than the seal diameter.
