- The following are questions asked by vendors regarding VA-250-11-RP-0312. There were several questions that needed clarification regarding the above solicitation. 1.Do the attachments need to be answer and attached with the solicitation? Answer: Yes, All attachments have to be answer and submitted with the solicitation. a.Contractor Certification. b.Key Personal and Temporary Emergency Substitution. c.Verification of Status of Apparently Successful Offeror. d.National Provider Identifier 2.On Standard Form 1449 block 28 is checked with an indication that 4 copies are to be returned to the issuing office. On page 11, paragraph 1.4.B it states that "Proposals are to be submitted electronically to Glen Johnson, Contract Specialist..." Please advise if you want four (4) hard copies of the proposal or one electronic copy. ANSWER: Please submit four hard copies that must be signed and one electronically signed 3.On page 11 of the solicitation, paragraph 1.4. D it states that "Price proposals are to be submitted using a similar type of excel spreadsheet provided as part of this RFP Document." Vendor did not find an excel spreadsheet within the solicitation. Would it be acceptable to provide the required information in the format identified under paragraph 1.3 Schedule of Supplies/Services? Please advise. ANSWER: Yes, this is acceptable. 4.On page 24 of the solicitation, paragraph (1) "The Contractor shall submit an original invoice and three copies (or electronic invoice, if authorized) to the address designated in the contract to receive invoices." If the contractor submits an invoice to the Financial Services Center via fax, will one copy be acceptable? Please advise. ANSWER: Please submit electronic version also 5.Why are 12 radiologists required for such low exam volume? Answer: 12 radiologists historically have provided coverage for the VA; you can propose a different number for your solicitation if you think that will meet our needs. 6.Is this a Small Business or Service Disable Veterans Owned set aside? Answer: Yes, This is 100% Service Disable Veterans Owned Small Business set aside only. 7.On the first page in block #17a for Code and Facility Code what goes in these blocks. Answer: These blocks will remain empty as they are used by the VA only. However, block 17a required the vendor to provide their full address, and telephone number 8.On page #1 of the solicitation, what boxes are we responsible for completing? Answer: Vendor is only responsible for completing boxes 17a, 30a, 30b and 30c. Vendor should use the schedule to provide their quotes for each type of interpretations. 9.Is it required that the radiologists be fellowship trained? Answer: No, the radiologists are not required to be fellowship trained. 10. Please define documentation required for employee credentialing of members with direct access to patient information. Answer: Documentation in accordance with JCAHO Policies 11. What are VHA training requirements for physicians? Answer: Board Certification, American Board of Radiology 12.VA Cyber Security Awareness and Rule of Behavior, where is the training performed and is the certification required as part of the current submission. Answer: This part of VA Directive 6500.6, which all personnel must completed training is provided online. Certification is not required as part of the submission, it will be provided. 13.VPN/BPG - Can the connection be made with cable connection in addition to T1 Lines? Is there an existing T1 line service to this site; is there a T3 line that can be shared? And what is the name of the phone and cable service provider to the location of requested service. Answer: This is site to site only, VPN, National VA Connection. There is no T1 line, it is to slow. There no T3 line to share. There is no phone or cable service provider. 14.Please provide a list of the current amendments for the RFP. Answer: The amendments are included in the solicitation 15.Request those vendors that classify themselves as "Self-Certified Small Disadvantage Business" produce documentation that they qualify. Answer: Vendor is providing documentation that they are a Service Disable Veteran Owned Small Business (SDVOSB) and have the seal in VetBiz that shows they have been certified. We will also get that verified. Anyone not verified will be considered non-responses. 16.Why isn't Joint Commission accreditation required? This signifies high standards for quality healthcare, to include physician credentialing and Quality Assurance Metrics. Answer: JCAHO is the standard. 17.Transcription - does vendor dictate exam reports to the VA transcriptionist and does vendor dictate the exams to their staff and transmit the final signed report to the VA? Answer: The vendor is to use voice recognition software such as Power scribe to put the Final Report into the VA Hospital Information System. The vendor transmits the Final Report into VA HIS. 18.Will equipment placed at the VA site as part of this contract be returned at its completion? This would include servers, switches and modems. Answer: The equipment will be returned. 19.Is there a fee to be a member of the hospital medical staff? Answer: There is no fee. 20.Vendor wishes to confirm that MRI protocols are not required at the submission of the proposal. Answer: MRI protocols are not required as the submission of the proposal. 21.Regarding the CT scan exam volume, please give an estimate by stratifying the period in hours during the 24 hour day. Answer: The workload is mainly nights and weekends. 22.Please define the factors of the Performance Work Statement. Answer: The vendor is to specify how the work is to be accomplished. 23.Is the set aside of SDVOSB vendors being enforced? Answer: Yes. 24.Does the VA Department of Medical Images have a list of what they define as "Critical Findings"? If so, may we have a copy? Answer: There is no current list. 25.What is the last date that a vendor may submit questions to the VA regarding this solicitation? Answer: Questions can be received up until January 13, 2012.
