Description

The National Cemetery Administration is soliciting offers under Solicitation No. VA-786-12-R-0058 for the following service: The contractor shall be required to provide bronze medallions for privately purchased headstones for cemeteries in the United States and Abroad within twenty (20) calendar days following the date the order is issued. The specifications for Bronze Medallion are as follows: Product CodeInscription Type M1VA Bronze Medallion (5 inch Diameter) M2VA Bronze Medallion (3 inch Diameter) M3VA Bronze Medallions (1 ½ inch Diameter) Potential offerors shall submit at contractor expense the samples and packaging recommendations for items which will be used to determine potential awardees ability to manufacture and deliver VA Medallions. The samples shall conform to government specifications for: color, field texture, pigmented finish, protective coating and lettering. The samples shall be clearly marked (on the back) with permanent marker to show the following: Contractor's name and solicitation number. The words "Property of the United States Government" shall be stamped on the back of each medallion sample. Medallion samples as follows: "The 5 inch medallion shall include: U.S. ARMY "The 3 inch medallion shall include: U.S. NAVY "The 1 ½ inch medallion shall be inscribed U.S. AIR FORCE Additional Submittals "Sample mounting hardware pack and epoxy required for the 5 inch medallion, including affixing materials (reference drawing) "Sample mounting hardware pack and epoxy required for the 3 inch medallion and 1 ½ inch medallion "Sample mailing/shipping package or envelope to be used "Sample of package insert (addendum 2) The samples shall be manufactured in the same facilities to be used in the manufacture of production items. If more than one production facility is to be used, one sample from each additional facility shall b provided. The samples shall be the basis for comparison of all medallions produced under this contract. Evaluation Criteria for this award will be in the following order: "Past Performance, "Technical Qualifications "Price This acquisition is 100% set aside for Service Disabled Veteran Owned Small Business (SDVOSB). The NAICS Code is 327991. Size standard is 500 employees. Electronic solicitations will be available for download from http://www.fbo.gov on or about February 03, 2012. Responses to the solicitation are due on or about 2:00 p.m., February 21, 2012. Any amendments to the solicitation will also be available at www.fbo.gov. Offerors are advised they are responsible for obtaining any amendments.
