Description
THIS IS AN AMENDMENT TO THE ORIGINAL SOURCES SOUGHT DOCUMENT, CORRECTING THE POINT OF CONTACT PHONE NUMBER. THE POINT OF CONTACT IS CHAD SANTONE AT CHAD.SANTONE@VA.GOV OR 509-434-7535. This is not a solicitation notice. In accordance with Federal Acquisition Regulation 19.1405 and VA Acquisition Regulations 819.7005, the Department of Veterans Affairs, Network Contracting Office 20, is conducting market research to find Service-Disabled Veteran-Owned Small Business (SDVOSB) or Veteran-Owned Small Business (VOSB) Concerns who are interested in an upcoming project and will definitely submit an offer. Responses from this notice will assist in determining if the acquisition should be set-aside for competition restricted to SDVOSB or VOSB Concerns. The project information is as follows: Project number: 531-313 Project Name: Construct Medical Imaging Building Project Location: Boise VA Medical Center, 500 West Fort Street, Boise, Idaho 83702 Project Scope of Work: Construct the Medical Imaging Building, at the Boise VA Medical Center, Boise, Idaho in strict accordance with specifications and drawings. The Medical Imaging Building will be a single story, approximately 5700 square foot, Medical Imaging facility to house one Government Furnished and Installed Magnetic Resonance Imaging (MRI) machine and associated functions. Also, the facility is designed and shall be constructed to accommodate a second 3.0 Tesla machine in the future. Work includes demolition of an existing connecting corridor between two existing buildings and establishing a new corridor connection from the new MRI facility to an existing building (67). The Contractor shall provide all supervision, labor, materials, and equipment for quality control, demolition, civil, utility, structural, electrical, mechanical, controls, asbestos and lead abatement, architectural, fire alarm and sprinkler, landscape, and other trades necessary to provide a complete and useable facility per project specifications and drawings. Construction activity shall include all required testing/inspections. All costs for this work shall be included in the submitted bid. The NAICS Code is 236220 and size standard is $33.5 million. In accordance with the Veterans Affairs Acquisition Regulations (VAAR) clause 836.204 the construction magnitude is between $2,000,000.00 and $5,000,000.00. You are requested to provide a positive response to include at a minimum: 1.Company name, address and phone number. 2.Verification your company is registered in VetBiz and is a certified SDVOSB or VOSB. This is a mandatory certification requirement if your company is claiming SDVOSB or VOSB status. 3.Written confirmation your company intends to submit an offer. (Request you only respond if you have a genuine interest in the project and will likely submit an offer.) 4.Company profile to include certified NAICS code it is registered for. 5.Proof your company has received a contract, conducts business, or has bid on, similar work in the general locality of the project. 6.If your company has not done business in the general locality, explain why your company is interested in the project and how you intend to overcome the obstacles of bidding and working in a new location (i.e. subcontractor and supplier plans, superintendence, project management, customer relations, etc.). 7.Bonding capacity and bonding information. Submit above information in detail and writing to Chad Santone via E-mail at chad.santone@va.gov no later than February 09, 2012. Incomplete information will not be considered or included in resultant set-aside determination. A synopsis will be posted with the set-aside information on the Federal Business Opportunities website at www.fedbizops.gov on or about March 15, 2012.
