Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice. Statement of Work - Overton Brooks VA Medical Center located at 510 East Stoner Ave. Shreveport LA requires the services of a contractor to provide all resources necessary to accomplish deliverables described in this statement of work (SOW), except as otherwise specified; install approximately 3125 total square feet of Earth Woods Collection LSI vinyl plank 4 inch by 36 inch; the color code number is 1012GM or Equal, in our public and employee dining areas. The contractor shall furnish and install rubber base boards 6 inch in the public dining and 4 inch in the employee dining area. V.A. will remove and reset all fixtures. The contractor shall remove the existing floor and prep for installation of new flooring according to manufactures recommendations. The contractor shall schedule the work required under this statement of work with Engineering Service within seven calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). All work shall be accomplished after normal business hours, and on weekends. Contract shall be awarded as Firm-Fixed-Price. Potential offerors are highly encouraged to attend the site visit scheduled for February 15, 2012 at 10:00 a.m. central time. Site visit may be coordinated with Fredrick Wilson, 318-990-4095, or by email at Fredrick.Wilson@va.gov. The location of the site visit will be 510 East Stoner Ave. Shreveport LA building 1, room 2W29. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation VA256-12-Q-0464 is to be issued as a 100% Small Business Set-Aside with offers from only small businesses sources being considered. U.S. Department of Labor Wage Determination 2005-2235 dated June 13, 2011 is hereby incorporated into this Solicitation and resulting award document. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-55. No telephone offers will be accepted. Only written offers received directly from the offeror are acceptable. The North American Industry Classification System (NAICS) Code is 238330. The size standard is $14 million.
