- Scope of work for Vision Services Request for Quote/Proposal IJCC 2012-13 Student Vision Care Services and Supplies The Iroquois Job Corps Center (IJCC) and Education & Training Resources (ETR); located at 11780 Tibbets Rd., Medina, NY 14103 are desirous of a fully insured contractor to submit a bid on the federally funded project mentioned herein; and if successfully awarded the subject contract, to satisfactorily perform the following work project according to the established criteria contained within same. P otential bidders must have or obtain a Dunn & Bradstreet (DUNS) number to be awarded a federal contract. Potential bidders must be registered in Central Contractor Registration (CCR) before being awarded a federal contract. Final bids are due to this center, not later than (February 23, 2012) at 4:00 PM. Point of contact for this project is the Purchasing Department. They can be reached via email at: tina.burrage@etrky.com Scope of Work 1. Vendors are needed to provide Student Vision Care Services and Products to the Iroquois Job Corps Academy. Please attach a Company services and supplies fee sheet and see these attachments advertised on the FBO.gov website. 2. All vision care services and supplies are for students who attend and reside at IJCC (11780 Tibbets Rd., Medina, NY, 14103), these services and products are to be furnished by the successful bidder (s). 3.
