Description: This a combine synopsis/solicitation for commercial service prepared in accordance with the format in Federal Acquisition Regulation (F.A.R.), Subpart 12.6 as supplement with additional information included in this notice. This announcement constitutes the only solicitation; proposals are begin requested and a written solicitation will not be issued. When determining best value, factors that will be considered are past performance, delivery terms, technical knowledge, and price. 1.Past Performance 10% 2.Pick-up and Delivery terms 30% 3.Technical Knowledge 30% 4.Price 30% The solicitation number is VA256-12-Q-0531 and is issued as an RFQ, submitted quotes will be emailed to Gary.Baker3@va.gov telephone quotes submitted will be considered a no bid. This will be an option contract a base with four option years in accordance with F.A.R 52.217-7 and F.A.R 52.217-9 submitted your bids in accordance to each year of contract service, with a complete ending total. Ensure that each submitted bid has the solicitation number on each page. The solicitation document and incorporated provisions and clause are those are those in effect through Federal Acquisition Circular 2005-55 The solicitation is being issued as unrestricted; the associated NACIS code is 562211. The size standards are unrestricted. This is a combine solicitation/synopsis is for the complete handing, transport, and disposal of infectious medical waste from multiple sites of the SLVHCS listed in the statement of work. PERFORMANCE STATEMENT OF WORK Southeast Louisiana Veterans Health Care System (SLVHCS) FY12 Infectious Medical Waste Service Contract SCOPE OF WORK: The contractor shall provide all labor, personnel, equipment, tools, materials, supervision, and other items and services necessary to provide complete handling, transport and disposal of infectious medical waste from the multiple sites of the SLVHCS listed in the statement of work. The vendor will provide/make available all appropriate documentation sufficient to satisfy the local and State of Louisiana Department of Environmental Quality (LDEQ) requirements for disposal of both regulated and non-regulated medical waste. SPECIAL REQUIREMENTS/SAFETY: The contractor is required to follow all Federal, State and Local requirements pertaining to the safe handling, transport and disposal of regulated and non-regulated medical waste. Specifically, compliance with all applicable Resources Conservation and Recovery Act (RCRA), Occupational Safety and Health Act (OSHA), LDEQ, and Department of Transportation (DOT) restrictions and guidelines must be obeyed. Unless specifically stated, the size and type of packaging/container used to accumulate and transport the medical waste will be at the determination and discretion of the contractor. Currently, 96/95gallon reusable plastic refuse containers are being used at two locations with the remainder of the sites using 43lbs capacity corrugated cardboard boxes. Containers regardless of size will meet all DOT requirements and have all required markings and labels. 1. FY2012 contract services for removal of infectious medical waste, once each week, from the Southeast Louisiana Veterans Health Care System main facility, 1601 Perdido St., New Orleans, LA 70112, for a one year period based upon anticipated annual use of 20,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles (96/95 gallon containers) as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 2. FY2012 contract services for removal of infectious medical waste, once each week, from the VA Hammond Outpatient Clinic, 1131 South Morrison Blvd., Hammond, LA 70403, for a one year period based upon anticipated annual use of 4,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 3. FY2012 contract services for removal of infectious medical waste, once each week, from VA St. John Outpatient Clinic, 247 Veterans Blvd., Reserve, LA 70084 for a one year period based upon anticipated annual use of 4,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 4. FY2012 contract services for removal of infectious medical waste, once each week, from the VA Slidell Outpatient Clinic at 60491 Doss Dr., Suite B, Slidell, LA 70460 for a one year period based upon anticipated annual use of 4,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 5. FY2012 Contract services for removal of infectious medical waste, once each week, from VA Mandeville Dental Clinic, 812 Park Ave., Mandeville, LA 70448, for one year period, based upon anticipated annual use of 4,000lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 6. FY2012 Contract services for removal of infectious medical waste, once each week, from VA Metairie Dental Clinic, 2901 North Causeway Blvd., Metairie, LA 70002, for a one year period, based upon anticipated annual use of 4,000lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 7. FY2012 Contract services for removal of infectious medical waste, on an on-call as needed basis, from VA Mental Health Clinic, located at 3500 Canal St., New Orleans, LA 70112, for a one year period, based upon anticipated annual use of 2,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 8. FY2012 contract services for removal of infectious medical waste, once each week, from the VA Baton Rouge Anselmo (Blood Lab), 7850 Anselmo Ln., Baton Rouge, LA 70801, for a one year period based upon anticipated annual use of 4,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 9. FY2012 contract services for removal of infectious medical waste, once each week, from the VA Baton Rouge (Essen) Clinic, 7986 Essen Ln., Baton Rouge, LA 70809, for a one year period based upon anticipated annual use of 4,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. 10. FY2012 contract services for removal of infectious medical waste, twice each week from the VA Ambulatory Procedure Unit (APU), 2237 Poydras St., New Orleans, LA 70112, for a one year period based upon anticipated annual use of 12,000 lbs. The vendor will furnish all resources to provide turn-key infectious waste disposal, including all equipment, labor and transportation. On-site receptacles (96/95 gallon containers) as needed by the facility, on or off-site processing as required, permanent final placement (landfill) facilities as required, and provide copies of all disposal manifests, in accordance with attached work statement and Federal, State and local laws and regulations. Oral communications are not acceptable in response to this notice; interested parties may identify their interest and capability to respond to this requirement no later than 2/23/2012, offers submitted past the appointed time shall be considered a no bid.
