- attach the new SOW from the last amendment: Pricing Schedule and Incorporated Clauses which updates the Basis of Award The purpose of this Amendment 2 is to: -extend the response date for quotes in the combined solicitation/synopsis, -change the pricing schedule and incorporated clauses which updates the basis of award -attach the new SOW from the last amendment This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is FA4830-12-Q-0002 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-55, Defense Acquisition Circular (DAC) 91-13 (Correction) and Air Force Acquisition Circular (AFAC) 2012-0104. The North American Industry Classification System (NAICS) is 813110. The small business size standard is $7.0 million. This acquisition will be set aside 100% for small businesses in accordance with Federal Acquisition Regulation (FAR) 19.502-2(a). Description: Work to be performs includes the contractor to provide all personnel and supervision necessary to perform duties required for the position of Protestant Religious Education Coordinator (PREC) as defined in this Statement of Work (SOW), attached. The Contractor shall perform to the minimum standards of performance in this contract. Additionally, the PREC shall be for all Protestant Religious Education for the base chapel, and shall work with the Chaplains in preparation and interpretation of policies, as well as in the application of procedures for the total program; shall advise the Chaplains on all areas of Protestant Religious Education program management, while adhering to the MRE 503, Court Martial Lawyer Military Rule of Evidence--confidential communication to the clergy and his/her assistant and staff. See Attachment 2, SOW for full description of the requirement. Quotations must be submitted on the attached Pricing Schedule Provision at FAR 52.212-1, Instructions to Offerors- Commercial Items applies to this acquisition. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF QUOTATIONS, NAME, ADDRESS; TELEPHONE NUMBER OF OFFEROR; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Contractors shall insert unit prices in the attached pricing schedule and sign below indicating prices to be used. By signing, the offeror agrees with all terms, conditions, and provisions included in the solicitation. Quotations shall also contain all other documentation specified herein. PERIOD OF ACCEPTANCE OF QUOTE: The offeror agrees to hold the prices in its quote firm for 30 calendar days from the date specified for receipt of quotes, unless another time period is specified in an addendum to the solicitation. LATE QUOTATIONS: Request for Quotations or modification of quotations received at the address specified for the receipt of quotations after the exact time specified for receipt of quotes will not be considered. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award based on the Lowest Price Technically Acceptable. See more information in the addendum to 52.212-2 in Attachment 1 Pricing Schedule and Incorporated Clauses. QUOTER MUST BE CENTRAL CONTRACTOR REGISTERED (CCR). Information concerning CCR requirements may be viewed via the internet at http://www.ccr.gov or by calling the CCR Registration Centers at 1-866-606-8220. All offerors must be Central Contractor Registered (CCR) and online representation and certification application (ORCA) must be activated. ORCA registration for on line representation and certification requirements may be viewed via the internet at www.bpn.gov. OFFERORS MAY INCLUDE A COMPLETE COPY OF THE PROVISION AT FAR 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS WITH THEIR OFFER, which can be downloaded from the internet or requested in writing via email to the Point of Contact (POC). Offerors that fail to furnish the required representation information via submission or ORCA registration, or reject the terms and conditions of the solicitation, may be excluded from consideration.
