- Notice of Proposed Contract Actions IAW FAR 12 and 13, RFQ VA-259-11-RP-0252 The Grand Junction VA Medical Center (GRJVAMC), Grand Junction, CO and Service Area Organization (SAO) West 19 Rocky Mountain Acquisition Center (RMAC) is contemplating an award of a Firm Fixed Price, Indefinite Quantity Contract for Low Vision Optometry Specialist Services for the GRJVAMC under a Full and Open Competitive status. The proposed contract will be issued for commercial firm fixed price contract types. The period of performance for these contracts will be one year, with four additional, one-year option periods from date of award, which is estimated for May 01, 2012. While price will be a significant factor in the evaluation of offers, the final contract award will be based on best value to include past performance. Request for Proposal VA-259-11-RP-0252 will be available for download today March 01, 2012. No telephone and/ or electronic offers and requests for this solicitation will be allowed. Offers are to be received at the VA SAO West NCO19 RMAC, 4100 E. Mississippi Ave., Suite 900, Glendale, CO 80246, no later than March 26, 2012, 4:00 PM Mountain Time. The NAICS Code is 62111. Additional Requirements: Contractors shall be required to meet the standards of the Joint Commission and to provide current Optometry (Low Vision Specialist) licensure and proof of insurance.
