Description

See attached document VA248-12-Q-1058_1. This is a combined synopsis/solicitation for a commercial supply and/or service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will NOT be issued. This is a Request For Quote (RFQ) and the solicitation number is VA248-12-Q-1058_1. This solicitation document and all incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-49. The relevant NAICS code is 325120 with small business size standards of 1000 employees. Quotes are due on 23 March 2012 at 10:00a.m. (EST). Quotes should be submitted to LaVerne Astroth by email (LaVerne.Astroth@va.gov) or facsimile (813-979-3637). The James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd, Tampa, FL 33612, has a requirement for liquid 02 and tank rental and oxygen fill services. All work shall be done in accordance with the attached Statement of Work. The VA intends to establish a firm fixed price contract. The period of performance is for 12-month beginning on the effective date of contract award, which is estimated to be on 1 April 2012. The provision at 52.212-1, (Instructions to Offerors-Commercial), applies to this acquisition. Successful offerors must be registered with the Central Contractor Registration (https://www.bpn.gov/ccr/default.aspx) and provide Reps and Certs either electronically online or in writing by requesting a form (FAR 52.212-3 Offeror Representations and Certifications- Commercial Items) from the Contracting Officer.
