Description

This is a SOURCES SOUGHT ANNOUNCEMENT ONLY by the Department of Veterans Affairs ("VA"), VISN 22, Network Contracting Office, concerning provision of Transitional Housing Services for Homeless Veterans in the VA Loma Linda Healthcare System. Housing and treatment Services shall be provided in accordance with a plan of care and by a licensed health care professional. Supervision shall be provided in accordance with VA policy and regulations. It is neither a solicitation announcement nor a request for proposals or quotes and shall not obligate the Government to award a contract. Requests for a solicitation shall not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Large and Small Businesses interested and capable of performing the work. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business HUB-Zone business agencies and (g) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. Description: The VA Loma Linda Healthcare System is seeking a contractor to assist with the provision of eight Community Based Residential Beds that provide care and treatment services for Homeless Veterans. The Contractor shall be responsible for admitting Veterans, assessing Veterans needs, conducting initial and ongoing assessments, care planning, coordinating services, supervision, evaluating the care and services provided and discharge planning. 1. Contractor services will be furnished to Veterans for whom such care is specifically authorized by the VA and are homeless with substance abuse and/or other mental health diagnosis; 2. The per diem rate established will be an all inclusive rate; 3. Care will include, but not be limited to: Supervised alcohol and drug free environment; 4. Active affiliation with Alcoholic Anonymous (AA) programs; 5. Supportive Social Service(s); 6. Structured and group activities; 7. Physical activities; 8. Individual and group counseling as required with special emphasis on vocational rehabilitation in collaboration with VA program resources; 9. Basic health and personal hygiene maintenance; 10. Storage and Monitoring of medications. The contractor's facility must have a current occupancy permit or license, as required by the authority that has jurisdiction to issue such. The Contractor must adhere to all applicable local, state and federal laws. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The North American Classification System (NAICS) code for this acquisition is 624229. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the Central Contractor Registration (CCR located on the web at www.ccr.gov). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://vip.vetbiz.gov.
