- Statement of Work Conduct a fire and life safety assessment of the Cheyenne VAMC and prepare required Joint Commission Statement of Conditions eSOC and paper documentation. The survey will include the main hospital facility, approximately 311,700 sq. ft., the warehouse Building, approximately 6100 square feet, Boiler Building, approximately 8000 square feet and the Electrical Distribution Building, approximately 600 square feet. As a minimum, the scope of services to include the following: 1.Conduct a survey of the Buildings in order to determine compliance with the 2012 edition of NFPA 101, Life Safety Code and Joint Commission. This survey will be used for the 2013 JCOPH certification. 2.Provide recommendations for the appropriate occupancy classification for each section of each building. 3.Conduct a random survey of fire and smoke barrier walls above the accessible ceilings. On redlined drawings, denote wall deficiencies noted for future repair. 4.While on-site, review the existing electronic Joint Commission data and assist in its update. 5.At the completion of the site survey, provide an exit conference with Chief of Facilities Management, Safety Manager, and Senior Leadership staff to discuss findings. 6.The following data shall be presented in a usable format of Microsoft Word, Excel or Access. A.General Building description including height, size, construction classification and occupancy. B.Information provided by the medical center regarding any equivalencies approved by the Joint Commission (if any). C.Small scale floor plan drawings showing the occupancy classification for each area. Also identify hazardous rooms and key note any special life safety features. The hospital will provide a CD containing the current electronic drawing files (in AutoCAD) and current fire and smoke barrier wall arrangements for review and re-plotting prior to the site survey. D.Recommendations on how to correct any non-compliant items that are not correctable by normal construction methods. E.Deviations from NFPA 101 and/or The Joint Commission Standards will be identified and listed by type. All deviations must reference the specific NFPA 101 section and/or The Joint Commission Standard Number. F.Prepare a preliminary Plan For Improvement (PFI) in the draft submittal for any items that are not correctable as normal punch list items. After receiving the medical center's input, complete the PFI for inclusion in the final SOC documents. 7.Three copies of the SOC documents will be submitted for review. 8.Provide 2 hard copies and 1 soft copy of the final draft and 1 computer CD containing the final SOC documents and drawings. 9.Minor deficiency lists shall be submitted within 2 weeks after completion of the site survey. Final documentation shall be prepared within two weeks after receipt of the final review comments. 10.The intent of this project is to receive a high quality SOC report, update the Fire Protection Features Drawings to reflect current healthcare provisions and PFI assistance using a standard Microsoft Word, Excel or Access format for the utilization of information (subject to VA approval). Survey will be conducted by the Safety Manager and a representative of the contractor's office experienced in the Life Safety Code and the Joint Commission SOC/PFI process.
