- Statment of Work (SOW) Price Element Breakdown Attachment 3- Past Performance Questionaire Attachment 2- Past Project Info Attachment 1- Additional Terms and Conditions Soliciation HSHQE5-12-R-00009 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All responsible sources may submit a proposal that if timely received shall be considered by the agency. The solicitation is issued as a Request for Proposal (RFP), HSHQE5-12-R-00009. This document and all attachments including the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-57. All proposals must be received no later than 5:00pm EST time on April 19, 2012 addressed to Rayshiena Mitchell as described in Attachment 1---Additional Terms and Conditions. All required information for proposal submission can be found in Attachment 1. In order to be eligible for award all offerors must be registered with the Central Contract Registration.
