Description
NOTE: THIS NOTICE IS NOT A SOLICITATION. THE SUBJECT SOURCES SOUGHT IS ONLY A REQUEST FOR INFORMATION AT THIS TIME. ANY SUBSEQUENT SOLICITATION WILL BE POSTED AT A LATER DATE. The Philadelphia VA Medical Center is currently seeking sources to furnish all parts, labor and transportation to provide preventive maintenance and repair for the following equipment located at the Philadelphia VA Medical Center: Varian Clinac IX Linear Accelerator, including 120 multi-leaf collimator, Portal Vision, Respiratory Gating, On-Board Imaging (OBI), and Rapid Arc. Service engineers must have certified service training and at least three (3) years of experience in both scheduled and unscheduled maintenance on the above identified equipment. Vendor must be able to maintain software connectivity/communication among the linear accelerator, Varian Eclipse Radiation Treatment Planning System, and Varian Aria Record/Verify systems. Any and all interested and capable vendors are requested to respond to this sources sought with an e-mail message of interest, and provide the following information: 1.Name and address of your firm. 2.Point of contact name and phone number at your firm. 3.Provide GSA contract number, if applicable to this requirement. 4.Socio-economic category of your firm under NAICS code 811219. 5.Staffing capacity and capabilities to meet above stated requirements. 6.The training and experience of your firm's employees. 7.Any other pertinent information regarding this requirement.
