Solicitation Number: VA241-12-Q-0014 Notice Type: Combined Synopsis/Solicitation Synopsis: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA241-12-Q-0014 and is issued as an RFQ. (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular (FAC) 2005-55, effective February 2, 2012. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as a sole source requirement. VISN 1 Contracting Activity intends to negotiate a sole source firm fixed price award under the authority of FAR Part 6.302-1, only one responsible source, to the vendor, American Medical Depot, a small business, and they are located at 4380 NW 135th St, Opa Locka, FL, 33054-4418 (approved Justification for Other Than Full and Open Competition is attached). However, all other responsible sources that feel they can provide this or a similar item may submit a capability statement, proposal, or quotation, which may be considered by the agency. The North American Industry Classification System (NAICS) code is 339113, and the size standard is 500 employees. (v) This requirement consists of the following line items: 1.Product Number: 0702-001-000 Neptune 2 Rover Ultra (120V) - Quantity of three (3) each 2.Product Number: 0702-014-000 Neptune 2 Docking Station (120V) - Quantity of one (1) each 3.Product Number: 0702-001-000W Procare Coverage - Neptune 2 Rover Procare Warranty - Quantity of three (3) each (COPY OF WARRANTY PLAN MUST BE SUBMITTED WITH QUOTE) 4.Product Number: 0702-014-000W Procare Coverage - Neptune 2 Docking Station Procare Warranty - Quantity of one (1) each (COPY OF WARRANTY PLAN MUST BE SUBMITTED WITH QUOTE) 5.Product Number: MANUALS Service and Operating Manuals - Quantity one (1) Set (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: The VISN 1 Contracting Activity intends to negotiate on a sole source basis pursuant to FAR 6.302-1 -- Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements for a firm fixed price award with American Medical Depot located at 4380 NW 135th St, Opa Locka, FL, 33054-4418 for equipment to include line items listed in Section (v) above. The VAMC Togus requires the procurement of the above equipment for disposal of items in the Operating Room during direct patient care for the safety of staff as they prepare for the next surgical case. This equipment is manufactured by Stryker and is only available from American Medical Depot. (vii) Delivery FOB Destination to: Department of Veterans Affairs VAMC Togus 1 VA Center August ME 04330 More information will be provided upon contract award. (viii) 52.212-1 Instructions to Offerors - Commercial Items, apply to this solicitation. (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror who is deemed the lowest bidder technically acceptable as a best value to the Government. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- Offeror's are to include a COMPLETED COPY OF PROVISION 52.212-3 WITH THE SUBMITTED QUOTE, unless offeror has a current ORCA certification.
