Description

The Department of Veterans Affairs, Central Alabama Veterans Health Care System, 215 Perry Hill Road, Montgomery, AL 36109 intends to negotiate a sole source contract under the authority of FAR 6.302-1, with Symon Coms Inc, 500 North Central Expressway, Suite 175, Plano, TX 75074 to provide the following equipment: DescriptionItem No. BAE On-site SW Installation/Support Services “ 32 hours 1500 Project Manager Installation Planning and Coordination Services 1571 SDA-830 8427 Travel and living expenses 8451 Upgrade to SES V 11 7899-11 The Central Alabama Veterans Health Care System currently has in use the Symon projection media player, and requires components that are compatible with the system. Any newly acquired components must be compatible with the existing equipment, software and accessories. This notice of intent is NOT A REQUEST FOR COMPETITIVE PROPOSALS. However, interested parties may identify their experience in providing this equipment by submitting the following information. (1) STATEMENT OF CABABILITY: Submit a brief description [three (3) pages or less] that shows your company can provide the equipment/system. Include past experience in providing the equipment to the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your ability to deliver as stated. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS relative to NAICS 334119, Other Computer Peripheral Equipment Manufacturing (size standard 1,000 employees). : (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.
