Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-12-Q-0172 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 423450 and the business size standard is 100 employees. Requestors shall state their business size on their proposal. Proposed items must be BRAND NAME OR EQUAL to the items requested below, as they must be compatible with Olympus equipment. All responsible sources may submit a proposal which shall be considered by the contracting officer. Requested Items: Line Item 1: Four (4) EA Flexible Video Rhino-Laryngo Scope, Gyrus Item number: ENF-VQ Line Item 2: Two (2) Sinuscope, 3mm Diameter, wide angle, zero (0) degree, 150mm, autoclaveable, Gyrus Item number: 830000 Line Item 3: Two (2) EA Sinuscope, 3mm Diameter, wide angle, thirty (30) degree, 150mm, autoclaveable, Gyrus Item number: 830030 Items shall be delivered 7 days ARO to the VA Ann Arbor Healthcare System, 2215 Fuller Rd., Ann Arbor, MI 48105. The provision at 52.212-1, Instructions to Offerors-Commercial applies to this acquisition and offerors must comply with all instructions contained within it. Award will be made to the lowest price, technically acceptable offer. The following additional provisions and clauses apply: 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 4, 6, 23, 26, 27, 28, 29, 31, 38, 42, and 48.. The following additional clauses apply, and upon request, the contracting officer will make their full text available: FAR 52.204-7, FAR 52.225-25, FAR 52.252-2, VAAR 852.211-73 and VAAR 852.270-4 Clauses Incorporated by References (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. The items called for by this RFQ have been identified in the schedule by a "brand name or equal" description. Such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of products that will be satisfactory. Quotes offering "equal" products (including products of the brand name manufacturer other than the one described by brand name) will be considered for award if such products are clearly identified in the quote and are determined by the Government to meet fully the salient characteristics requirements listed above.
