- SYNOPSIS DESCRIPTION: The Department of Veterans Affairs, Network Contracting Office (NCO) 19 Rocky Mountain Acquisition Center, 4100 East Mississippi Avenue, Suite 900, Glendale, CO 80246 intends to solicit sources to enter into one or more firm-fixed price indefinite delivery, indefinite quantity contracts to provide the necessary personnel, labor, equipment and transportation services each week-day for pick-up and delivery for the facilities within the VA Salt Lake City Health Care System (VASLCHCS). The contractor shall deliver dry ice, and pick-up/deliver pharmaceuticals, supplies, x-rays, lab specimens, patient records, office supplies, forms, and other items as required by the VASLCHCS. The contractor shall provide a driver and vehicle for pick-up and delivery. Services shall be for a one-year base period with four one-year pre-priced option periods for renewal. The solicitation package will be available for download on or about April 20, 2012. Proposals submitted shall be due no later than 4:30 p.m. mountain time on May 18, 2012. The applicable NAICS code is 492110 with a small business size standard of 1,500 employees. This solicitation is a 100% Small Business Set Aside. The solicitation reference is VA-259-11-RP-0115 - Courier and Dry Ice Delivery Services - VA Salt Lake City Health Care System (VASLCHCS), and should be referenced in all correspondence/inquiries. The locations for service to be provided are as follows: VA Salt Lake City Health Care System, 500 Foothill Drive, Salt Lake City, Utah 84148; Ogden VA Community Based Outpatient Clinic, 982 Chambers Street, South Ogden, UT 84403; Pocatello VA Community Based Outpatient Clinic, 444 Hospital Way, Suite 801, Pocatello, Idaho 82601; Elko VA Primary Care Tele-Outreach Clinic, Gateway Center #1, 2715 Argent Avenue, Suite #2, Elko, Nevada 89801; Idaho Falls Primary Care Tele-Outreach Clinic, Eagle Pines Plaza, 3544 East 17th Street, Suite #102, Idaho Falls, Idaho 83401; Orem VA Community Based Outpatient Clinic, 1443 West 800, North, Suite #302, Orem, Utah 84770; Nephi VA Community Based Outpatient Clinic, Central Valley Medical Center, 48 West 1500 North, Nephi, Utah 84648; and Price Primary Care-Tele-Outreach Clinic, 600 West 189 South, Price, Utah 84501.
