Description

Dental Hygienist Statement of Services Project: On-Site Dentist Hygienist "THIS IS A SUBCONTRACTING OPPORTUNITY" The Little Rock Job Corps Center operated by Adams and Associates located at 6900 Scott Hamilton Dr. Little Rock, Arkansas has 300 + students enrolled in our residential and non-residential training facility. Little Rock Job Corps is currently soliciting request for quote for the following on-site medical services: Medical Services Hours per week Number of weeks Center Hygienist 12 48 The interested parties must include the following information with their proposal: •A. Dental Hygienist $________ hourly rate for 12 hours per week for 48 •B. Resume •C. References •D. Current medical & DEA licenses, if applicable, for the State of Arkansas All proposals are due on May 7, 2012 by 5:00 p.m. central time.
