Description
MARKET RESEARCH FOR HOME OXYGEN SERVICES: The purpose of this announcement is to gain knowledge of potential qualified sources and their size classification (hub zone, 8(a), small, small disadvantage, service disabled veteran owned small business, or large business) relative to NAICS Code 532291 (size standard of $7.0 Million). Reponses to this notice will be used by the Government to make appropriate acquisition decisions. All interested offerors will have to respond to this notice in order to be considered. The Department of Veteran Affairs, Network Contracting Activity, VISN 22, will award a contract to provide Home Oxygen supplies for five different VA Hospital Systems - Southern Nevada (NV), Greater Los Angeles (CA), Long Beach (CA), Loma Linda (CA), and San Diego (CA). The intended contract period will be for a Base Year and four Option years. Summary of Products and Services to be solicited: The Contractor shall manage Government-Furnish Equipment rental oxygen equipment by providing storage and maintenance of high-pressure cylinders, concentrators, oxygen refill systems, emergency back-up systems and supplies to eligible VA beneficiaries as required by the hospital systems. The Contractor shall provide delivery and set-up of VA patients. Services provided shall be all inclusive of deliveries, transportation, and delivery/set-up of equipment, in-service training for beneficiaries/ care providers to include patient education, safety inspections and Preventive Maintenance Inspection (PMI) service appointments. All services shall be provided either at Healthcare System, Veteran's residence, or in a nursing home where the veteran resides. The Contracting Officer's Technical Representative (COTR) or other personnel specifically authorized in the contract (hereafter authorized personnel), will notify the Contractor, fax, or email to initiate service; the beneficiary's oxygen prescription, the equipment, supplies and services to be provided, including date and place of delivery. (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if you firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm certified "Hub zone" firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or veteran owned small business?
