- Performance Work Statement Summer seasonal grounds Maintenance VA MEDICAL CENTER, GRAND JUNCTION COLORADO This scope of work provides for the summer maintenance of grounds at VA Medical Center, Grand Junction, Colorado. Work includes, but not limited to, mowing, Fertilizing, Pruning, Sprinkler system repairs, Trash collection, Cleaning of parking lot gutters, leaf removal, Serenity Garden (weekly hand weeding), Weed control as requires, etc. Summer seasonal grounds maintenance Period of Performance is from April 1-October 31, 2012. All work shall be accomplished in a professional manner and in compliance with the latest federal, state and local codes and regulations. DESCRIPTION 1. Mowing all turf areas weekly. 2. Edging Monthly. 3. Fertilizing Five (5) required per season (MSDS to be provided before application) 4. Pruning Twice (2) per season or as required. 5. Sprinkler system start-up, system blown-down and winterization, Repairs as required during the season. 6. Trash collection from exterior containers three (3) times weekly, no less than (10) containers. 7. Cleaning of parking lot & roadway gutters shall be performed twice (2) a Season. May 15th and September 15th. 8. Serenity Garden to be hand weeded weekly. 9. Weed control applications to be as required through-out the season (MSDS required on all products being used on site). 10. Leaf removal/Treatment shall be at the end of the growing season. 11. Pressure-washing of both east and west main entry concrete walkways at least four (4) times starting on April 15th every 60 days. General Service Provider shall submit a schedule of all procedures during the contract season. Schedule shall indicate all mowing, trimming, trash containers, pressure and other tasks as noted in this scope. The Service Provider shall for review and approval, submit all chemicals, compounds and other materials intended to be utilized in the performance of this contract along with their appropriate MSDS Sheets. All Service Provider personnel shall be identified by a personalized badge, worn visibly, displaying personal name and Service Provider. Service Provider employees working at the facility shall be familiar with Medical Center Fire, Hazardous materials and Disaster policies. (Copies provided by facility Management). Equipment parking to be assigned by VA Medical Center as required, Trash Dumpsters to be provided be VA. Service Provider shall provide at least two (2) Contact numbers to the VA Medical Center.
