- The National Cemetery Administration is soliciting offers under solicitation number VA-786-12-R-0147 for I1, Blank Marble Columbarium Niche Covers, for the following cemetery: Jacksonville National Cemetery 4083 Lannie Road Jacksonville, FL 32218-1247 The period of performance will be from date of award through 30 calendar days. This acquisition is 100% set aside small business. The NAICS Code is 327991. Size standard is 500 employees. Awardee shall provide all labor and materials necessary to supply (1000 ea) blank marble columbarium niche covers for use as replacements. Replacements shall be comparable in color and shading to the existing marble niche covers mounted on columbaria at Jacksonville National Cemetery. Sources from different states are acceptable if the MPS, Program Support Unit's evaluation of a sample confirms that a suitable match will result for color, visual texture, finish, and testing standards. For aesthetic purposes, the supplied material shall match existing material having characteristics of marble from Vermont or Colorado. All potential offerors shall submit at their expense one (1) blank marble niche cover according to the specifications listed in the RFP by the closing date of this solicitation. Sample shall be representative of the stone to be used, the finish to be attained and the placement and diameter of the holes to be drilled. Testing results for the sample shall be submitted per the RFP by the closing date of the solicitation. The sample shall be marked clearly on the back with indelible marker, showing contractor name, solicitation number, quarry source of stone and name of stone. Any sample which arrives damaged will not be acceptable. Additional instructions will be included in the RFP. Samples shall be received by Memorial Programs Service (MPS) prior to closing date and they will become the property of the Department of Veterans Affairs and will not be returned to vendors. Full size copies of drawings are available upon request at no additional charge to potential vendors. The request should be e-mailed to the NCA Memorial Programs Service (MPS) Program Support Unit at MPSProgSupport@va.gov and should include the contractor's name, mailing address, phone number, e-mail address and the name and drawing number of the drawing(s) being requested. Evaluation criteria for this award will be in the following order: 1) Price; 2) Technical Qualifications, and 3) Past Performance. Electronic solicitations will be available for download from http://www.fbo.gov on or about April 27, 2012. Proposals are due on or about 2:00 P.M. on May 11, 2012. Any amendments to the solicitation will also be available at www.fbo.gov. Offerors are advised they are responsible for obtaining any amendments.
