- Attachment 4 Attachment 3 Attachment 2 Attachment 1 Solicitation SF-18 FBO Amendment 1 uploads the solicitation documents. To be considered responsive, quotations shall include: - A completed SF-18 - A completed Section K - A technical proposal in response to the criteria identified in Section M. Offerors shall clearly write their name and address, the solicitation number (AG-569R-S-12-0018), and date and time due on their sealed envelopes. Address quotations to: Lake States Acquisition Team, Attn: Shawn Lacina, 500 Hanson Lake Road, Rhinelander, WI 54501. All quotes, mailed or hand carried, must be received by the date, time and location specified in this solicitation. Telegraphic, electronic and/or facsimile quotes are not allowed on this solicitation. The Chequamegon-Nicolet National Forest announces an opportunity for the 18 Mile Culvert project. In accordance with FAR Part 36.204(b), the estimated magnitude of construction is between $25,000 and $100,000. The applicable North American Industry Classification System (NAICS) Code is 237310. The Small Business Administration size standard is $33.5 million. Contractors are required to be actively registered with the Central Contractor Registration (CCR) system and the Online Representations and Certifications Application (ORCA). THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. No reimbursement will be made for any costs associated with providing information in response to this synopsis, the pending solicitation or any follow up information requests. The solicitation will be posted to the FedBizOpps website on or about April 27, 2012. No hard copies of the solicitation, supporting documents and applicable amendments, if any will be provided. To receive the entire solicitation package and any applicable amendments, prospective offerors must download them from the FedBizOpps website. The contractor shall provide all management and project administration services, quality control, logistical support, vehicles, labor, equipment and materials to complete the project on time and on schedule. The intent of this contract is to provide for the replacement of the Eighteen Mile Creek culvert on medium volume Forest Road 213; traffic control monitoring; furnish and install the new culvert; furnish and install backfill for the culvert; furnish all sub-grade material for raising of the road grade; furnish and place aggregate to final road grade; install permanent bridge signs; seed and mulch all disturbed areas; and protect the Eighteen Mile River and associated wetlands from siltation and other destructive impacts of construction.
