- Wage Determination Drawings Submittals Special Project Requirements Spefication Sheet Solicitation SF-18 This FBO ammendment 1 uploads the solicitation documents for this requirement. See solicitation for pre-bid meeting details. To be considered responsive, quotations shall include: - A completed SF-18 - A completed Section K - A technical proposal in response to the criteria identified in Section M. Offerors shall clearly write their name and address, the solicitation number (AG-569R-S-12-0016), and date and time due on their sealed envelopes. Address quotations to: Lake States Acquisition Team, Attn: Shawn Lacina, 500 Hanson Lake Road, Rhinelander, WI 54501. All quotes, mailed or hand carried, must be received by the date, time and location specified in this solicitation. Telegraphic, electronic and/or facsimile quotes are not allowed on this solicitation. he Chequamegon-Nicolet National Forest announces an opportunity for the Thornapple River Culvert. In accordance with FAR Part 36.204(b), the estimated magnitude of construction is between $100,000 and $150,000. The applicable North American Industry Classification System (NAICS) Code is 237310. The Small Business Administration size standard is $33.5 million. Contractors are required to be actively registered with the Central Contractor Registration (CCR) system and the Online Representations and Certifications Application (ORCA). THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. No reimbursement will be made for any costs associated with providing information in response to this synopsis, the pending solicitation or any follow up information requests. The solicitation will be posted to the FedBizOpps website on or about April 27, 2012. No hard copies of the solicitation, supporting documents and applicable amendments, if any will be provided. To receive the entire solicitation package and any applicable amendments, prospective offerors must download them from the FedBizOpps website. The contractor shall provide all management and project administration services, quality control, logistical support, vehicles, labor, equipment and materials to complete the project on time and on schedule. The intent of this contract is to provide for the replacement of 2 - 48" concrete pipe culverts at the Thornapple River crossing of Forest Road 162; furnish and install new aluminum box culvert structure with full invert, toe walls, headwalls, and wingwalls; furnish and install backfill; furnish and place aggregate to final road grade; install permanent object marker signs; seed and mulch all disturbed areas; and protect the Thornapple River and associated wetlands from siltation and other destructive impacts of construction. The Contractor shall furnish all labor, materials, equipment, tools, transportation, and supplies necessary to meet the intent of this contract; and shall perform all work in close conformity with the drawings and specifications, and in accordance with the provisions of the contract.
