Description

This amendment #0001 is hereby issued to provide the following information. 1. To correct the response due date to May 2, 2012 at 2:00 p.m. EST. 2. To add the following: Rental agreement for ARC safe uniforms, approximately 308 shirts and 308 pairs of pants. Size range from the following: Shirts - small to 5Xlarge, pants - waist 30-56. All garments must comply with NFPA 70E and HACCP Standards. For base year from date of award through twelve months, four option years for twelve months each. NOTE: For evaluation purposes, three shirts and three pairs of pants must be delivered to Angela Taylor (EMS), VAMC Dayton, Bldg 126, 4100 W. Third St., Dayton, OH 45428 no later than 2:00 p.m. EST May 2, 2012.
