- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4877-12-Q-0388 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58, Defense Circular Number 20120420, Air Force Acquisition Circular 2012-0330. This procurement is being issued as Full and Open. Short description of the requirement: Paramedic Training Course for 3 personnel PLEASE SEE ATTACHED STATEMENT OF WORK FOR FULL REQUIREMENT. Contractor shall provide all necessary tools, equipment, materials, transportation, personnel, supervision, management, and any other requirements needed. CLAUSES Offeror must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer.
