Statement of Work and Technical Evaluation Criteria CLINICAL SUPPORT FOR THE 2011 AND 2012 UPDATES TO THE CLINICAL LABORATORY FEE SCHEDULE AND ANALYSIS OF MOLECULAR PATHOLOGY PROCEDURE TEST CODES Solicitation# CM-393-2012-0040 This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a firm fixed priced purchase order for "Clinical Support for the 2012 and 2013 Updates to the Clinical Laboratory Fee Schedule and Analysis of Molecular Pathology Test Codes" The period of performance will be July 8, 2012 through July 07, 2013. The North American Industry Classification Code (NAICS) is 621511. The purpose of this contract is to procure the services of a contractor with a medical background who has the expertise, knowledge, experience, and skills required to: 1) Provide clinical knowledge of laboratory services and the ability to assist CMS in determining payment rates for new laboratory services: 2) Provide clinical knowledge of molecular pathology procedures; 3) Attend and participate in all meetings related to the Clinical Laboratory Fee Schedule, as well as the Annual Public Meeting for new laboratory test codes; 4) Attend and participate in all meetings related to gathering information concerning molecular pathology procedures; 5) Assist CMS in responding to public comments regarding CMS preliminary and final decisions on payment rates; and 6) Work with CMS staff to ensure that payment rates for new and existing Clinical Laboratory Fee Schedule codes are set at accurate levels consistent with the law. The Statement of Work/Deliverables and Technical Evaluation Criteria are attached. This Request for Quotes (RFQ) is being set aside for small business concerns. Responses must be sent electronically in MS Word format to Lucille.Lee@cms.hhs.gov no later than 11:00am on June 5, 2012. Responses shall be no more than 15 pages and shall include the following: 1) Technical Approach; 2) Key Personnel and 3) Past performance information - relevant experience to perform the required services. All responses will be evaluated on a best value basis. The government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price (whole dollars); the best value (considering price and other factors). All responses/quotes submitted shall remain valid for a period of 180 days. For the purpose of establishing timely receipt of a quotation, the time will be established based the electronic submission. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict or the appearance of a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated. Please submit proposals in whole dollar amounts only. All questions must be submitted electronically by May 29, 2012. Questions will not be answered by the telephone.
